Behaviour change is often overrated as the ultimate solution to improve health because without social, economic & structural environments that facilitate rather than obstruct healthy choices, behaviour change remains a privilege reserved for those who have the resources to remove the obstructions.

The system uses primary care as a cheap offload for what the state doesn’t want to pay for. It’s too easy. GPs across the country aren’t perfect, be we are generally very good at absorbing patients care needs for cheap. But it’s our staff getting screamed out when the wider service doesn’t work

I think we are closer to this than ever before. Let’s face it, patients value their GP, but governments do not. They see medicine as something practiced in hospitals. Our secondary care colleagues are sadly part of this, everything seeming to have to go to a specialists (I found AF pls ref to cardio

The strength of GP is continuity of care and "being the expert in knowing your patients", compared to being an increasingly siloed specialist. Losing that continuity has led to ACPs doing more of the role of GP. Continuity of care is overwhelmingly popular with patients and GPs.

And continuity is also overwhelmingly associated with far better outcomes for patients. I remain to be convinced by any claims that AI will revolutionise medicine. It smacks of the Emperor's new clothes.

NEW: White House says the arrest and deportation of student activist & legal US resident, Mahmoud Khalil, is a blueprint for more. The final line here is chilling. The White House specifically says Khalil was not breaking the law. No evidence, no due process, no crime. He was just disappeared.
Indeed, a White House official told The Free Press that the basis for targeting Khalil is being used as a blueprint for investigations against other students.

Khalil is a "threat to the foreign policy and national security interests of the United States," said the official, noting that this calculation was the driving force behind the arrest. "The allegation here is not that he was breaking the law," said the official.

WHAT is going on at Twitter UK, part of the business empire of Elon Musk, the world's richest man and the Donald's "First Buddy", whose salutes and focus on the birth rate gap increasingly echo Peter Sellers' Dr Strangelove?

Companies House last week filed a "First Gazette Notice", warning the UK arm of Musk's social media group X could be compulsorily struck off and dissolved in two months' time. No reason is given, but most likely is the failure to file the company accounts to December 2023, due last 31 December. However, the notice was ahead of itself – dated 4 March, the date of proposed publication in the official Gazette!

The notice as filed warns that if Twitter UK is dissolved its property and assets are deemed "bona vacantia" and pass to the Crown. Surely fake news. But if true, presumably Musk the Merciless will not be so busy sacking US government employees to allow this to happen. Perhaps it was caused by one cut too many at Twitter UK?
