Bizarrely the US with separation of church and state has its politics entirely dictated by religion. Whereas UK which is a theocracy actually has minimal real control over its legislatures by religion, despite bishops sitting in the HoL.

We need more useful RCTs. Eg comparing ibuprofen and naproxen for gout… so many patients come to me having nearly fixed themselves with simple ibuprofen.

You have to suspend MPs who break the three line whip. There’s really no choice. They still voted against it despite the concessions.

10 YEAR PLAN - Neighbourhoods DOI: We aim to fit into the new multiple neighbourhood provider (MNP) model, and our member practices and PCNs have major stakes in the single neighbourhood provider (SNP) model The Plan is a mixed bag of good stuff and areas that need tightening for delivery. 1/17

Behaviour change is often overrated as the ultimate solution to improve health because without social, economic & structural environments that facilitate rather than obstruct healthy choices, behaviour change remains a privilege reserved for those who have the resources to remove the obstructions.

The system uses primary care as a cheap offload for what the state doesn’t want to pay for. It’s too easy. GPs across the country aren’t perfect, be we are generally very good at absorbing patients care needs for cheap. But it’s our staff getting screamed out when the wider service doesn’t work

I think we are closer to this than ever before. Let’s face it, patients value their GP, but governments do not. They see medicine as something practiced in hospitals. Our secondary care colleagues are sadly part of this, everything seeming to have to go to a specialists (I found AF pls ref to cardio

I’m sorry, but whoever wrote the sentence that NHS “care should… [be delivered] digitally by default” needs to be nowhere near healthcare, ever.
The plan makes clear that in future, the more prevention-focused NHS will operate on the basis that "care should happen as locally as it
can; [be] digitally by default; in a patient's

The strength of GP is continuity of care and "being the expert in knowing your patients", compared to being an increasingly siloed specialist. Losing that continuity has led to ACPs doing more of the role of GP. Continuity of care is overwhelmingly popular with patients and GPs.

And continuity is also overwhelmingly associated with far better outcomes for patients. I remain to be convinced by any claims that AI will revolutionise medicine. It smacks of the Emperor's new clothes.

Exactly. Without the GPs in high numbers it will just spread us further than we are. Then lengths they will go to in order to not give GP surgeries funding is incredible

Eg one or two neurologists in Lincs. Do we want them driving hours between Skegness, Boston, and Lincoln, (not seeing patients) or using that time to see more patients. It’s a tough balance.

Neighbourhood centres will likely increase costs and could fragment care. There’s a reason why Darzi centres never materialised… until now

Phil Williams @scottishphil83.bsky.social · 03/07/2025
Ironically this feels like it kills off continuity of care for “your GP” which is the opposite of the rhetoric. Addiction to AI same week as Lincs ICB told all GPs to stop using AI scribe software. Make it make sense! Unified health record has possibilities.

Mental health A&G is a huge no no. We only need specialist help if we’ve tried and failed several strategies already. Often recommended complicated multi drug regimens. Need specialist follow up and assessment of effect. Antipsychotics IMO should never be initiated in general practice.

Gov addicted to AI in the week GPs in Lincs were told to stop using all AI scribe software. 🤦‍♂️ it’s great but it’s not going to save the NHS. And trusts / practices need extra funding for these eye wateringly expensive licenses - higher than equivalent staff costs.

Phil Williams @scottishphil83.bsky.social · 03/07/2025
I’m just sad. ICBs don’t have a good track record for listening to patients. The plan talks about enlarging GP areas - risking continuity which we know saves lives. Even having acute trusts run general practice when they struggle to run themselves.

Children in particular are being killed by the political unwillingness to stand up to the car lobby. Clean air zones save lives. But politicians would rather babble on about “AI”.

Huge issues with extra liability being dumped on GPs though. DH and CQC expect us to action any issues on data even not owned by us. Eg pt goes to UTC and BP is high (likely stress). Day before BP at GP is perfect. We have to repeat BP otherwise affects our quality.
