Through additional analyses (metabolite effects, GRS PheWAS, MR) we drill down on the discordant risk mechanisms, identifying liver fat accumulation as a nexus of discordant risk. But not all liver fat is equal, different mechanisms of hepatic fat accumulation come with different disease risks.

To try and understand these patterns we took a variant clustering approach (bNMF) which has been useful for partitioning T2D-risk pathways using GWAS variants. But here, we cluster shared variants, and identify finer-scale clusters than what has previously been observed for T2D.

Excited about a new preprint from the lab looking at pleiotropic variant effects between type 2 diabetes and coronary artery disease. In our analysis we identify 42 unreported shared loci, but most of them (35) have discordant effects on these co-occurring diseases! www.medrxiv.org/content/10.6...

Clinical statements (AHA, ESC) have suggested that PRS may be most useful as a risk-enhancing factor for those at intermediate risk. We used PRS to reclassify individuals at intermediate risk by QRISK3 and showed improved sensitivity and fairness benefits (particularly for age & sex).

We evaluated PRS in two contexts, as a standalone risk predictor and as a risk-enhancing factor. Given the known performance differences across ancestries, we were surprised to find that the fairness of PRS predictions across ethnicities was equal to or lower other CVD risk factors when used alone.
