At a conference 2 yrs ago I presented some of my work on reporting of race and ethnicity for clinical trials. A prof told me that he doesn’t think my research is worth pursuing b/c reporting doesn’t matter. Here’s a JAMA IM editorial explaining why it does. jamanetwork.com/journals/jam...
At JAMA Internal Medicine, we have become increasingly aware that some US researchers are unable to report on race or ethnicity as demographic variables to describe their sample or to conduct stratified analyses by these variables. In some cases, we have been informed that based on 3 recent US federal executive orders, -3 the stewards of federal databases, such as those responsible for distributing data from the Centers for Medicaid and Medicare Services (CMS), are unable to release any race or ethnicity data for research purposes. The restrictions may be particularly prohibitive when study investigators are US federal employees. We have been informed of similar concerns occurring at other biomedical journals. The potential for restricted access to race and ethnicity data across many federal databases has been high-lighted, 4 with disruptions reported during 2025 at the Behavioral Risk Factor Surveillance System (BRFSS) and other Centers for Disease Control and Prevention (CDC) databases, the US Census database, Health and Human Services (HHS), and National Institutes of Health (NIH) public use databases. At JAMA Internal Medicine, we have become increasingly aware that some US researchers are unable to report on race or ethnicity as demographic variables to describe their sample or to conduct stratified analyses by these variables. Without information on participants' racial or ethnic backgrounds, it is difficult to assess whether study results are generalizable across diverse populations or whether important differences in disease risk, treatment response, and health outcomes exist among subgroups." For example, disaggregating mortality data on Asian American and Pacific Islander individuals showed higher all-cause and cause-specific mortality rates among Pacific Islander adults that would not have been apparent if the data were not reported.'2 Another example is a recent review highlighting the disproportionate impact of metabolic dysfunction-associated steatotic liver disease on the Hispanic or Latino population. '3 The authors emphasize the need for disaggregating this larger group to capture the cultural nuances of specific groups to best apply effective treatment strategies.
This lack of reporting may also obscure health disparities, hinder efforts to address inequities, and reduce the ability of clinicians and investigators to develop or evaluate evidence-based interventions that will be effective across all populations. Moreover, nonreporting of race or ethnicity will preclude the ability to conduct meta-analyses or data syntheses that combine results by racial or ethnic subgroups across multiple studies or trials. Such analyses have been critical to generate adequate power to uncover therapeutic differences across racial and ethnic subgroups and to understand specific mechanisms to improve treatments that may be more effective across all populations. One example is the discovery of variation in the effectiveness of clopidogrel and proton pump inhibitors among some Southeast Asian populations, which was based on meta-analyses of genetic variants. 4 Generalizability to all subgroups is crucial because these meta-analyses o…Thus, transparent and consistent reporting of race and ethnicity is essential for continued scientific progress that benefits all of us. Understanding effects and outcomes across racial and ethnic subgroups is important for improving scientific rigor, promoting health equity, and ensuring that biomedical research benefits all. Calling out the serious adverse consequences and implications of these executive orders on medical progress is our responsibility as medical editors and health care professionals.
