Only reason to sequence the RASi first is that they are good antihypertensives (but note the BP effect in the combo arm of #confidence) #nephjc But no matter the sequence, the goal should be to have someone on the four pillars in less than 6 months

In 2009 as a nephrology trainee I asked a wise old nephrologist what his practice was regarding CKD-MBD as I was confused. His reply “well Chris I mostly ignore it unless the values are awful because there isn’t a shred of evidence for any of it…” Turns out he’s still right!

just learned this & mind blown 🤯 CKD-EPI, MDRD etc give us GFR corrected for body surface area (ml/min/1.73m2) BUT we're supposed to use the *absolute* GFR (ml/min) for drug dosing (eg KDIGO 2024 guidelines below; FDA agrees) in obesity, the computer's ml/min/1.73m2 can be way too low #EMIMCC
