PO meds are so hot right now (PO=IV) but in critically ill patients (esp with enteral feeding tubes), the pharmacology gets tricky & data is often scant when the patient starts vomiting... do we redose everything? IMHO the default route for live-saving meds in ICU should still be IV #EMIMCC

Continuous Kidney Replacement Therapies: Core Curriculum 2025

Continuous kidney replacement therapy (CKRT) has largely become the standard of care in developed nations for the treatment of acute kidney injury in patients with shock, acute brain injury, acute liver failure, and other forms of critical illness. A Core Curriculum article in the American Journal of Kidney Diseases reviews the physicochemical principles of CKRT, describes when and how to prescribe CKRT, and summarizes trials that serve as the foundation for approaches to initiation timing, dosing, vascular access, and anticoagulation for CKRT. The authors highlight components of high-value multidisciplinary care for patients receiving CKRT, including drug dosing, nutrition, physical rehabilitation, and CKRT quality assurance programs.
