'Recovery' was always a big problem for the PACE trial, to the extent that they didn't actually mention it at all in the main trial paper in 2011. Discussion of it was relegated to a later paper in 2013, which also contained a metric tonne of fudge, because they knew they were in trouble over it.

A guide to the Biopsychosocial model recently put out by Long Covid Advocacy ultimately brought me to a 2005 book chapter, written by Professor George Davey Smith, entitled The biopsychosocial approach: a note of caution in Biopsychosocial Medicine: An Integrated Approach to Understanding Illness, edited by Peter White and published by Oxford University Press. The chapter is available from a University of Bristol website. I thought the divergence of opinion between Profs Davey Smith and Wessely was sufficiently interesting to write this short commentary.

Digestion is, IMO, the most important focus of improving care for people with severe ME. Without proper digestion, nutrients are not utilized and medications are not metabolized. There are multiple mechanisms by which digestion potentially fails/slows. 🧪 #SevereMEDay 🧪
GI dysmotility is associated with mortality in critical illness[1] and multimorbidity analyses.[2]
Gut issues affects electrolyte availability and can lead to dehydration[1]
Malabsorption due to gut endothelial damage [4]
Deficiency of nutrients that require gut cofactors e.g. B vitamins, leading to issues with neurotransmitters like dopamine, serotonin, & slowing a plethora of metabolic processes
If digestion cannot ‘keep up’ with metabolism, circulating nutrients are chronically low: starvation at a cellular level

Abstract
Background
Many patients with Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) have significant care needs. However, post-exertional malaise—the defining feature of ME/CFS—means that even minor physical, orthostatic, cognitive, or sensory stressors can trigger a disproportionate worsening of symptoms. This results in specific requirements and significant challenges in home care. Care is still provided predominantly by family caregivers, who frequently lack adequate assistance and support. At the same time, there are significant gaps in knowledge, care infrastructure, and professional guidance for nurses and other healthcare professionals, as well as physicians involved in providing care.

Objective
The objective of this guide is to structure care measures in a way that prevents overexertion and promotes stability.

Methods
The guide is based on a compilation of practice-oriented measures that have proven effective from the perspective of patients and family caregivers. These were professionally categorized and further developed by experts in nursing science, physical therapy, general medicine and public health.

Results
The guide describes how to adapt key dimensions of care—from nutrition and personal hygiene to communication and managing emotional stress—to disease-specific exertion thresholds. Additionally, it outlines requirements for the caregiving relationship and the planning of home visits and discusses the application of palliative care principles.
