My American cultural geography take: The Midwest spans an area from Bismarck, ND, Pierre, SD, Lincoln, NE, and Wichita, KS in the West all the way to and including Buffalo, NY and Pittsburgh, PA in the East -- and exists north of I-70 in Missouri, and north of the Ohio River east of the MO/IL line.

In my book Coverage Denied (and in an article in JHPPL), I refer to coverage denials as “rationing by inconvenience” precisely because care isn’t rationed through final denials, but through the imposition of red tape that patients and physicians struggle with because it’s very taxing to overcome.

Incredible parallels in this Berinsky & @gabelenz.bsky.social paper. Politicians didn't stand up to Joe McCarthy in large part because they incorrectly inferred McCarthy/ism was extremely popular. Not standing up to McCarthy was a kind of 1950s Popularism gated academic.oup.com/poq/article/...
