Randomized controlled trials are the gold standard for establishing cause-and-effect relationships and attributing health outcomes to a specific intervention rather than other factors. Five such trials have been conducted in the USA, UK, Denmark, and Japan to investigate the effect of UPFs on obesity and related outcomes (5–9). The results have been highly publicized and widely communicated, often in ways that suggest that ultraprocessing is inherently harmful to health (1, 2). The design of these clinical trials, however, makes it very difficult to attribute the unfavorable effects of UPF-rich diets to ultraprocessing per se. Instead, these effects are highly likely to be due to differences in traditional nutritional properties that frequently—but not uniformly—co-occur in UPFs (see the figure).
All of these trials used a crossover design, in which the same participants consumed UPF-rich and non-UPF diets in a random order. All foods and drinks that participants consumed were provided in either inpatient (involving overnight stays at a hospital, clinic, or research center) (5, 6) or outpatient (7, 8) settings, or through a combination of supervised and unsupervised eating (9). This “feeding” trial design prevents confounding from varying diet adherence that occurs in “diet advice” studies. In three trials, participants consumed the diets at will until comfortably full (ad libitum) (5–7), whereas in two trials they received a fixed amount of calories (8, 9). The duration of each diet arm of the study was 1 to 3 weeks in four trials (5, 6, 8, 9), with the longest lasting 8 weeks (7).

Time for my yearly update on how we're doing in AoNZ vis-a-vis PhD stipends (poorly). I use the Marsden Fund PhD and Masters stipends as the benchmark because they are at the *top* end of what is offered, btw. 🧪
