We suggest that highly-adhesive, α-toxin-defective isolates promote inflammatory but self-limiting infections, whereas weakly adhesive, high-toxicity strains favour immune evasion and severe disease.

Our findings define adhesion as a polygenic and evolutionarily variable trait with genetic variants in known adhesins and novel loci all affecting adhesion capacity.

Functional analyses reveal that high levels of SpA limit ClfA-dependent fibrinogen binding, CsbB interferes with ClfA exposure on bacterial surface, and GlcA enhances fibronectin binding via metabolic regulation.

We applied a bacterial GWAS to identify genetic variants in S. aureus associated with differences in bacterial adhesion. The GWAS identified genetic variants in genes encoding known adhesins (clfA, fnbA, fnbB, ebh), other surface factors (spa, sdrH), MGEs, and other loci (csbB, glcA).

We found that stronger fibrinogen-binding, particularly in strains lacking α-toxin, correlated with heightened systemic inflammation but lower mortality, linking bacterial adhesion to distinct clinical outcomes.
