Living with Long COVID means your body stops playing by the rules you’ve known your whole life. Weakness, instability, and sudden loss of strength aren’t “just fatigue” or something you can push through, they’re the result of real, measurable damage happening at a physiological level.

I don't like to talk about covid in wastewater as cases much beyond "line go up means cases go up", and there is sooo much confusion in distinguishing detail like the difference between asymptomatic (no symptoms) and pre/post symptomatic (no symptoms at test time), but here is an observation 1/9

It might feel like the days of Covid are well behind most of us, but six years on from the initial outbreak, around 200 thousand New Zealanders are still dealing with it because they have Long Covid. This does not even begin to account for the immense human suffering long COVID inflicts. COVID is bad. Long COVID is bad and you get it from getting COVID. Don’t get COVID. Wear an N95 respirator in indoor public spaces. Get vaccinated. Clean the air we all share.

The first, an electronic cohort study of over 600,000 active duty military personnel, found that 42% had at least one symptom more than 30 days after contracting COVID-19. “Mild to moderate cases of this syndrome could leave military personnel with long-term deficits in their overall fitness,” the study’s authors wrote. Then, a Centers for Disease Control and Prevention preprint that assessed survey data at the state level found around 7% of U.S. adults were affected by Long COVID.
A small study shared as a preprint in bioXriv found SARS-C0V-2 spike protein in the gut tissues of people with Long COVID. Eight people with Long COVID and five controls underwent colon biopsies for the study. Researchers found spike protein in both cohorts, but more in Long COVID, as well as evidence that it caused immune dysregulation in Long COVID compared to controls. “This is one more piece of hard evidence that antigen persistence is a legitimate driver of symptoms and organ system dysfunction for Long COVID and vaccine injury,” co-author David Putrino wrote on social media.
Researcher Dianna Arnoux-Whiteman was awarded a $45,000 grant to survey the effects of Long COVID in the Blackfeet Nation. With funding from a Montana research grant, Arnoux-Whiteman plans to find an accurate count of people with Long COVID in the Blackfeet Nation, as well as work with the community and healthcare workers to improve awareness and care for the disease. The president of Arnoiux-Whiteman’s college told Flathead Beacon: “It will not only support more responsive care and resources within the community but also offer insights that may guide other Native communities…as they respond to the emerging realities of Long COVID diagnoses.”

Since I spent years in infectious disease research, and have followed the excess deaths literature closely, I was fascinated by a new Science Advances paper (Kiang et al., PMID: 41849604).

It mostly confirms what many of us already thought—but really nails it down in a novel way.

Using individual-level death data (not just excess mortality), the authors estimate ~19% more COVID deaths than officially reported in the U.S., with underreporting especially large early in the pandemic.

The key point: most of those “missing” deaths were likely misclassified COVID deaths, not indirect effects like lockdowns or patients avoiding care.

That matters, because excess-death analyses always left some wiggle room for debate*. This approach narrows that uncertainty.

Also not surprising—but now clearly demonstrated: the undercount wasn’t random. It disproportionately affected poorer, less educated, and minority populations.

So the same inequities that shaped outcomes also shaped what got counted.

Bottom line:
This doesn’t change what I’ve believed for several years—it strengthens it.

*Before this study, the strongest evidence that most excess deaths were due to the virus itself was timing: waves of excess deaths tracked COVID cases and wastewater signals, not policy changes or other hypotheses. Counties with fewer excess deaths were generally those that followed scientific guidance. That was compelling—but indirect. This paper provides much more direct evidence.

A recent scientific paper compared excess death rates among countries, using the pre-pandemic period 2015-2019 as a baseline. A few years ago I integrated the same dataset they used (STMF from HMD) and built my own excess death analysis. I was reassured to see quite similar results. 🧵

Which is what I’ve been saying. It was a deeply inequitable decision for rich countries with strong health systems and lower HIV/malaria/malnutrition rates to decide to keep a pet SARS virus. Frankly, it’s just another form of oppression.

As you can see XFG is getting big in New York, while NB.1.8.1 does the same in California. Ontario/Canada has a decent amount of both. In Europe XFG is big in Catalunya/Spain, while NB.1.8.1 is a bit bigger in France then XFG. We'll now more in a few weeks! 2/2

In younger people such as Todd, complaints of brain fog and cognitive difficulties may be intertwined with, or stem from, other common long-Covid symptoms, such as fatigue and pain, doctors say.
Dr. Gabriel de Erausquin, a neurologist at University of Texas Health San Antonio, recently published a study that found the risk of moderate to severe cognitive impairment in older adults with long Covid was double that of younger ones.
For those long-Covid patients over 57, the symptoms associated with cognitive difficulties look much like early dementia, he says.
