It was a brief line in Healey's speech which I missed, but he said that from the next spending review, HMT will "ringfence prevention spending that brings down costs in the future". All depends on the definition, but could have *big* implications eg for public health/active travel budgets.

No, me neither, and of course metaraminol was off the table for obs when I did it. Phenyleph is also frantically expensive - when there was an NA shortage ITU pharmacy bills went through the roof…

We’re trying to move to prefilled metaraminol syringes here. Haven’t used phenyleph for ages but anything that needs double dilution before you can use it seems a bit of a risk (though I worked in one place that had prediluted ampules their pharmacy made).

Public Health England found that 4 in 10 adults aged 40 to 60 had not taken a brisk 10-minute walk at any point in the last month. I have been thinking about it a lot. For those people, attempting to walk or cycle will be physically painful. Of course they cannot imagine the use of a cycle lane.

The mistake is going out to public consultation on these schemes. There should be some necessary technical schemes, but lets stop asking every Tom Dick and Harry. Nothing ever gets done. The return on investment should be enough. This is like implementing a smoking ban pub by pub.

In other countries, it's quite common for people to live in flats above the #HighStreet - apart from pride in the place they live this also means there are people around in the evening & businesses to serve them. Compare with the desolate wasteland which is so many British high streets after 1800?

Creating a successful High Street in the age of internet shopping takes time, but we know how to do it: 1. Make it pedestrianised but accessible (in many places that will include access by car as well as bike and public transport).

All 3 services are in a bit of a mess, and it does seem that we get less for our spending than other countries. However, we're already at a point where we haven't enough frigates to meet our commitments, and the 23s that are left are increasingly worn out.

There’s a “yes, but…” to that - primary care colleagues would say that fear of loss of license might deter people from seeking treatment. Agree though, a “medical episode” should result in license suspension pending a proper independent medical assessment.

This is of course one of the things with 15 minute cities - if you have services within a short distance, then it means people don’t have to drive to access them (and more than that, people don’t get into the habit of driving to access them).

Problem is what to do - current system largely replies on self certification, which people don’t do because they fear the loss of independence because we don’t offer adequate alternatives to driving.
