In neonates with anhydramnios due to #fetal #kidney failure, serial #amnioinfusions before 26 weeks improved early survival outcomes, but many survivors required ongoing dialysis or kidney transplant and experienced significant morbidity. ja.ma/4wjibTR

T2e Kidney biopsies are recommended if CNIs have to be continued > 2 yrs in children, to exclude toxicity. Side-effect profiles may drive CNI choice as gingival hyperplasia & hypertrichosis are more prevalent with cyclosporine & glucose intolerance occurs more frequently with TAC. #NephJC

I do the same with any agent. Treat for 2 years then try off. If relapse, then another 2 years. #NephJc I don’t think there’s great data. Just seems a good balance of risk.
