Thus, to experience pain properly, the brain requires a well established, well balanced network architecture in which bottom-up input and top-down context are properly integrated. An imbalance in network level processes may also have connotations for abnormalities in how pain is processed

8/ The takeaway: even trauma symptoms below the clinical threshold seem to reshape how prefrontal and brainstem circuits work together in chronic pain, and they do it through the emotional side of pain, not the physical. Maybe we should stop treating trauma and pain as separate problems.

7/ A subregion dissociation sharpened it further: dlPFC–dl/lPAG connectivity tracked arousal/reactivity symptoms; dlPFC–vlPAG tracked avoidance. This maps onto known active vs. passive coping roles of these PAG columns.

6/ Behavioral relevance: these patterns tracked depression and pain catastrophizing, but not pain intensity or disability. PTSS appears to act on the affective dimension of pain, suggesting changed affective regulation.

4/ Task connectivity showed complementary effects. Higher PTSS scaled with stronger dlPFC–periaqueductal gray (PAG) coupling during task. Crucially, this was specific to PAG. No effect for amygdala or hippocampus, pointing to descending modulatory circuitry rather than general affective processing.

3/ Under the highest load (3-back), higher PTSS predicted reduced activation in left dlPFC and left IFGpo, core lateral prefrontal control regions. The effect was load-dependent: absent at low demand, emerging only when executive demand peaked.

2/ 53 chronic low back pain participants did an N-back task during fMRI across four loads (0- to 3-back). We tested PTSS both dimensionally (PCL-5) and as a high/low split, and asked whether trauma load shapes task-evoked activation and connectivity.
