Day 1 empiric therapy: sampling (culture) before therapy, clinical exams, consider alternate diagnosis;

Indications for extended courses: endocarditis (4-6 weeks), brain abscess (6-12 weeks), spondylodiscitis (6-12w), prosthetic joint infection (6-12 w), incomplete removal abscess, empyema, retained prosthesis: individualized

Day 5-7: 

if Patients NOT responding or positive cultures: reassess treatment, individualize according to source control, immune status, high risk microbiology (S aureus, candida), possible suboptimal therapy (MDR), intravascular devices, ? role of serial PCT for stopping early questioned

I know this preprint is basically rage bait, that's why I'm not linking it. But how cynical is it to say there is a death deficit after the pandemic that has been "paid off" by vulnerable people during the pandemic and excess deaths therefore are overestimated.

PS Papers where basic science and clinical data, in particular trial data, are all mixed together almost impossible to pull off well (and it's hard to find the right journal and audience). More often than not, you end up with a Frankenpaper.

This is a really interesting study -- ~1/3 of MPXV tests estimated to be false positive in DRC outbreak. MPXV is a DNA virus and it's very easy to pick up environmental DNA during PCR. Do we need to adjust testing protocols?
