This study is arguing that most of the price increases are for the cost of care and not greedy insurers: jamanetwork.com/journals/jam...
This cohort study, which analyzed state-level data on insurance premiums, health spending, and insurer markups from 2011 to 2024, found that mean insurance premiums grew by $3143 (78.4%), health spending grew by $2844 (84.2%), and insurer markups decreased in proportional terms from 18.6% in 2011 to 14.9% in 2024. Overall, the growth in health spending accounted for 91% of the overall growth in mean premiums.

Nearly 1 in 3 hospitals are now also insurers. Writing in @theconversation.com, USC Schaeffer’s Grace Mackleby discusses the good, the bad and the unknown of what this growing trend could mean for costs and patient care.

What happens when your hospital is also your insurance company? We’re starting to see the effects
Hospitals are tasked with making patients better, but when the hospital that provides care also owns the insurance company that pays for that care, the incentives get more complicated.

Honestly, this is one reason I think a lot of management strategies imported from the corporate world struggle in universities: the task of managing a university is invariably herding a lot of cats because only strange cats decide that teaching and researching at that level is what they want to do.

I am exaggerating somewhat here, of course, but also the old stereotype that academics tended to be a little bit weird is...pretty well based in fact. You have to be willing to absolutely devote yourself to a single field of study, which is a strange decision to make!

Forthcoming in the AER: "Difference-in-Differences with a Continuous Treatment" by Brantly Callaway, Andrew Goodman-Bacon, and Pedro H. C. Sant'Anna.
aeaweb.org
Difference-in-Differences with a Continuous Treatment
(Forthcoming Article) - This paper analyzes difference-in-differences designs with a continuous treatment. We show that treatment-on-the-treated-type parameters are identified under a parallel trends assumption analogous to the binary treatment case. However, comparing these parameters across treatments is challenging because parallel trends does not rule out selection bias. We discuss alternative, typically stronger, assumptions that eliminate selection bias. We further show that popular two-way fixed effects estimands admit multiple interpretations, depending on the underlying causal building block, all having important limitations as meaningful summaries of treatment effects. Finally, we introduce alternative estimation procedures that avoid these drawbacks and demonstrate them in an empirical application.

My guess is that there is enormous graft going on in federal contracting right now. These processes are less visible than grants in some ways, especially if there are no-bid contracts because of claims of an emergency. So when we see examples like this people should be concerned.
For example, a Texas law firm just received a $150 million one year grant to represent 24,000 children in immigration court. It has 26 employees. And does not specialize in immigration. The firm was co-founded by two Trump appointees.

ICE just paid $2.2 billion to purchase four prisons from a contractor, who will then still run the prisons for them. The whole point of contractors should be that you avoid making long-term capital investments that tie you down.
