Bottom left image is a great visual for trainees to learn and remember. If the bowel loops aren’t extending out to the abdominal wall on any side and seem like it’s centralized - need to look w US to assess for Abdominal ascites. Air filled bowel loops floating on intraabdominal fluid/ascites.

It is dangerously cold this weekend and soon we'll have a ton of snow. If you live in an area impacted by this snowstorm and you order delivery, you need to tip your driver at least 50%. They're putting themselves at great risk being out when it's this cold and hazardous.

That last point. Cassidy really has enormous leverage if he wants to use it. He would win this fight. Worst case is he winds up losing a primary and spends his 70s (he's 68 now) as a hero to millions. He'll likely never have as much influence over anything again as he has over this right now.

3 infant #pertussis deaths this year in Kentucky. "Of the three infant deaths reported over the past 12 months, all three were not vaccinated, nor were their mothers." We first tested maternal vaccination for whooping cough over 80 years ago. It's safe & effective & might have saved these babies.

J Pediatr. 2025;286:114719 I had thought this was a settled issue, Neil Finer showed many years ago that atropine alone decreased bradycardias during intubation. But as the authors of this new study point out, there is very little (or no) data about atropine as part of an intubation cocktail in the newborn. I have a bit of a beef with the introduction which suggests that the Kelly and Finer trial mentioned above was limited, as it did not "follow recommended premedication protocols".
