5) The primary outcome was the Brief Pain Inventory-Short Form (BPI-SF), which measures not only pain but also the impact on various activities and aspects of life. The treatment group improved by 0.5 out of range of 10 points, which wasn't statistically significant.

Now they're initiating trials using anti-CD38 and anti-CD14 antibodies, with some promising results. This one was unfortunately quite a lot, referenced a large number of publications-- sorry I could not hunt them down fast enough! Remember the recording will be out there in a week or so, likely.

1) Very sad news that Jo Cambridge has passed away. She was a Prof. in rheumatology who pioneered B-cell depletion therapy in autoimmune diseases together with Jonathan Edwards. But she also did several useful studies on ME/CFS and was much loved in the community.

1) Unfortunately, another negative result for immunoadsorption for Long Covid. A small trial that focused only on patients with elevated levels of antibodies against β1/β2-adrenergic and/or M3/M4-muscarinic acetylcholine receptors found no improvement in objective outcomes.

2) Inebilizumab targets the CD19 a marker on B-cells; the immune cells that produce antibodies. Compared to Rituximab (which targets CD20), inebilizumab targets a broader range of B-cell subsets, including early B cells and plasmablasts.

1) 🇩🇪 There's now more info about the PIONEER study which will test inebilizumab (a drug that targets B-cells), in a subgroup of ME/CFS patients. It's a randomised trial at the Charité that aims to include 38 participants.

2) The treatment trial is led by Carmen Scheibenbogen and will be a follow-up study to the randomized Phase II trial of the CD19 antibody inebilizumab. Tafasitamab also targets CD19 but is less expensive, so they want to test if this also works in a subgroup of 38 patients.

1) 🇮🇹 An Italian hospital showed reduced nerves in the gastric lining of Long Covid patients compared to controls. It was a preliminary study though, with only 12 patients and 8 controls.

2) The graph below shows the main findings. Unfortunately, there wasn't a placebo control group only a group that received no drug, only usual care. The primary outcome was the Fatigue Assessment Scale (FAS, range: 10-50), with higher scores indicating worse fatigue.

1) 🇬🇧 The results of the big STIMULATE-ICP study are in. It tested three repurposed drugs in hundreds of Long Covid patients: - colchicine: an anti-inflammatory - famotidine and loratadine: antihistamines - rivaroxaban: an anticoagulant Sadly, all three had poor results.

ABSTRACT
Background
Low-dose naltrexone (LDN) has been used off-label for fibromyalgia syndrome (FMS) for over a decade, supported by small crossover trials. The INNOVA study evaluated the safety and efficacy of LDN 4.5 mg versus placebo on pain intensity over 12 months in women with FMS.

Methods
In total, 98 women with FMS were randomised to LDN 4.5 mg/day (n = 48) or placebo (n = 50). The primary outcome was change in pain intensity (NRS 0–10) from baseline to 3 months in the intention-to-treat population. Secondary outcomes included functional impairment (FIQR), anxiety-depressive symptoms (DASS-21), cognitive impairment (MISCI), disability (WHODAS 2.0), pathological worry (GAD-7) and impression of change (PGIC/PSIC), assessed at baseline and at 3, 6 and 12 months. Perceived treatment allocation and safety were also assessed.

Results
At 3 months, mean change in pain intensity was −0.33 points with LDN and −0.64 with placebo, with an adjusted between-group difference of 0.49 (p = 0.236, d = 0.19). Secondary outcomes showed minor and inconsistent changes, with low and similar responder rates (p = 0.219–0.954). Exploratory analyses suggested some improvement among participants who believed they had received LDN, although effects were inconsistent. Adverse events, predominantly mild and transient, were reported by 33 (68.8%) participants in LDN and 36 (72%) in placebo, and no treatment-related serious events occurred.

Conclusions
LDN was well tolerated but did not demonstrate a clinically meaningful benefit over placebo for pain-related outcomes, underscoring the need for more effective pharmacological treatments.

Significance Statement
This RCT provides the first long-term evidence on the efficacy and safety of LDN in FMS. LDN (4.5 mg), administered as an add-on treatment, has a favourable safety profile, but the findings indicate that it does not produce clinically meaningful improvements in FMS symptoms compared to placebo at short- or long-term follow-up. 18) Overall, this is an impressive, in-depth paper providing evidence for autoimmunity in a subset of LC patients. But a decisive result of identifying a specific antibody that is pathological and causes symptoms, wasn’t achieved (yet).

10) The strongest results in the paper are the transfer of antibodies to mice who then developed symptoms such as pain sensitivity. Similar findings have been reported by the groups of Jeroen Den Dunnen, Charles Nicaise and Andreas Goebel.

8) One of the top hits was antibodies against the Mediator Complex Subunit 20 (MED20) which is involved in turning genes on and off. The paper lists findings suggesting these antibodies are pathologic such as reduced IgM/IgG ratio, complement deposition, etc.
