Does the study break down the case types for >85? With more minimally invasive surgical techniques and the increased use of noninvasive monitoring (BIS & Pulse variability index), I can see why fewer arterial lines are used. Anecdotally it feels that age has lower risk and ortho trauma surgeries.

Paradigm shifts in medicine are always characterized by resistance from within the medical community. See Ignaz Semmelweis, Joseph Lister, Louis Pasteur, etc. What we're witnessing is a paradigm shift in how metabolism and obesity are understood. It's not a surprise some doctors are resistant.

A quote about Nazi Germany that feels very relevant to current events.
“Each act, each occasion, is worse than the last, but only a little worse. You wait for the next and the next. You wait for the one great shocking occasion, thinking that others, when such a shock comes, will join with you in resisting somehow… But of course this isn't the way it happens. In between come all the hundreds of little steps, some of them imperceptible, each of them preparing you not to be shocked by the next.“

A lil tip for the folks with shoe polish on their lips today :)
Excerpt from Timothy Snyder's "On Tyranny:"

"1. Do not obey in advance."

"Most of the power of authoritarianism is freely given. In times like these, individuals think ahead about what a more repressive government will want, and then offer themselves without being asked. A citizen who adapts in this way is teaching power what it can do."

Public humiliation is still a common teaching tool in medical education. Here’s how it leaves patients worse off
Many doctors become reluctant to ask questions or seek help when uncertain. This is exactly the opposite of what most patients want in a doctor making crucial decisions about their care.

Gisèle Pelicot is an absolute boss. There is no end to the respect I have for her. The strength and dignity she has shown in the face of what must be a devastating and torturous personal trauma, is awe inspiring.
It is obvious that Gisèle Pelicot is very tired. Since September, she has sat in court most days listening to graphic descriptions of the alleged rapes of which she was the victim, and has withstood some aggressive questioning from defence lawyers.
Today, one of the last questions that was put to her had to do with her choosing to keep the Pelicot name despite divorcing Dominique Pelicot.
She answered that she wanted her grandchildren to be proud of it - because it would be associated with her and this trial, rather than her ex-husband's crimes. More than 100 people were sat in the courtroom, yet you could've heard a pin drop. Gisèle may be exhausted and carrying an enormous amount of trauma, but her voice was calm and very assured as she said this. It was also her first acknowledgment of the global resonance this trial has had.

A British pensioner whose bland sandwiches gained him unexpected fame in China has died. Keith Brown, a retired engineer known on Chinese social media as
"Old Dry Keith", became a hit when his Chinese wife Zhang Jian began to post videos of him assembling the ham and cheese sandwiches he liked to eat. Chinese observers were grimly fascinated by his dry, boring sandwiches, and his gentle, stoic manner - and he built up a cult following. "We go from questioning the dry old man, to understanding the dry old man, to becoming the dry old man," observed one cultural commentator.
"The old man is us, and his dry lunch is our dry life."
