The only way you know when AI Is giving wrong advice is because you know it’s wrong advice. That’s what experience gives you. And if you use AI to replace all the years of gaining experience, you’ve lost everything

…confident following our advice. This applies just as much to life prolonging treatment as to perhaps less momentous decisions eg about going on the OCP or having a knee replacement. The value of experience is incalculable. As an aside AI is a serious threat because…

We’re not taught enough about this, and as our own understanding is poor it’s no surprise we struggle to convey information accurately to patients…there is also the balance of wanting to present genuinely ‘expert’ opinion (we do have expertise) in a way that leaves our patient…

@jomorrison.medsky.social it was such a good question and I think you rightly point to a much wider issue regarding how medics feel about treatment. We know that Drs since the time of Hippocrates have overestimate benefits of treatment often by a dramatic factor eg 20-fold- and underestimated harm…

Let’s talk about death?
I am all for cafe culture and, with my addiction to caffeine, I have been a frequent visitor to Rosarios to put the world to right over a cup of black Italian coffee. There are many things to talk about in conversation with friends and family, but the end of your life isn't usually one of them! That's when I heard about the Death Cafe - an idea born in Switzerland but quickly spreading to London and around the world. We are not talking about a counselling session but a group get-together for the purpose of talking about the subject over food and drink.

Couldn’t agree more - it’s now a routine part of my outpatient assessment as well as inpatient. Patients understand it’s an evolving document and their wishes can change. We do it together and put it on Somerset’s fab electronic TEP there and then. Much relief.

I think I got a bit fierce about the so called ‘frailty index’ as created in various poor iterations for use in cardiovascular trials. Not the FI as a geriatrician knows it.. I didn’t say fraud but others have 😬

Good question. I’d base this on family/carer report. If they feel dementia drugs aren’t giving a meaningful improvement, best stop. They can worsen incontinence agitation unsteadiness. Agree re domino ad but this was average result. Some wd be better on Rx, others no difference. Is that sensible?
