Dear Dr. Brenner:
It is with a heavy heart that I have decided to resign from FDA and retire from federal service as Director of the Center for Biologics Evaluation and Research effective April 5, 2025. I leave behind a staff of professionals who are undoubtedly the most devoted to protecting and promoting the public health of any group of people that I have encountered during my four decades working in the public and private sectors. I have always done my best to advocate for their well-being and I would ask that you do the same during this very difficult time during which their critical importance to the safety and security of our nation may be underappreciated.
Over the past years I have been involved in enhancing the safety of our nation's blood supply, in advancing the field of cell and gene therapy, and in responding to public health emergencies. In the last of these, during the COVID-19 pandemic I had the privilege of watching the vision that I conceived for Operation Warp Speed in March 2020 in collaboration with Dr. Robert Kadlec become a reality under the leadership of HHS Secretary Azar and President Trump due to the unwavering commitment of public servants at FDA and elsewhere across the government. At FDA, the tireless efforts of staff across the agency resulted in remarkably expediting the development of vaccines against the virus, meeting the standards for quality, safety, and effectiveness expected by the American public. The vaccines undoubtedly markedly reduced morbidity and mortality from COVID-19 in the United States and elsewhere. Many of these same individuals applied learnings from the pandemic during a flawless response helping…ongoing multistate measles outbreak that is particularly severe in Texas reminds us of what happens when confidence in well-established science underlying public health and well-being is undermined.
Measles, which killed more than 100,000 unvaccinated children last year in Africa and Asia owing to pneumonitis and encephalitis caused by the virus, had been eliminated from our shores. The two-dose measles, mumps, rubella vaccine regimen (MMR) using over the past decades has a remarkably favorable benefit-risk profile. The MMR vaccine is 97% or more effective in preventing measles following the two-dose series, and its safety has been remarkably well studied. Though rarely followed by a single fever-related seizure, or very rarely by allergic reactions or blood clotting disorders, the vaccine very simply does not cause autism, nor is it associated with encephalitis or death. It does, however, protect against a potential devasting consequence of prior measles infection, subacute sclerosing panencephalitis (SSPE), which is an untreatable, relentlessly progressive neurologic disorder leading to death in about 1 in 10,000 individuals infected with measles. Undermining confidence in well-established vaccines that have met the high standards for quality, safety, and effectiveness that have been in place for decades at FDA is irresponsible, detrimental to public health, and a clear danger to our nation's health, safety. and security.
In the years following the pandemic, at the Center for Biologics Evaluation and Research we have applied the same unwavering commitment to public health priorities to the development of cell and gene therapies to address both hereditary and acquired rare diseases. During my tenure as Center Director we have approved 22 gene therapies, including the first gene therapy ever to be approved in the United States. I can tell a patient is autistic because you ask what their pain is from 1-10 while they're in excruciating pain from kidney stones and they're like "this is entirely subjective, how would I even know what a 10 feels like if I've never experienced it?" and every neurotypical person just tells you 11

New pseudoscience from the CDC website, courtesy of chief idiot HHS secretary. The evidence supporting Vitamin A treatment is largely in countries where the average person may be suffering from vitamin deficiencies. The primary recommendation of the CDC should be MMR vaccination, full stop.

I like how different medical specialties have different "vibes". I'm still pretty torn on EM vs psychiatry, and thinking a bit about IM or FM... but I love my EM colleagues, it's like a specialty just for ADHD types like me. The tox community is full of the nicest brilliant nerds you'll ever meet.

Most interesting thing I've seen recently was a young patient with thyrotoxicosis secondary to unmedicated Graves Disease. HR of like 140, most recent labs had TSH of like 0.01, so anxious they were about to burn a hole in their bed from shaking. I felt so bad for them!

My prereqs are from a decade ago, so just getting an interview is really reassuring - means at least someone is taking me seriously. I just wish this process wasn't so grueling and soul crushing. Having to wait a year between cycles with very limited feedback stinks.

I disagree. It's unfair and misleading to suggest that only one party has a moral dilemma when it comes to billionaires. Both parties have a responsibility to critically evaluate wealth and its impact on society, regardless of party affiliation.

TIL tuberculosis can infect more than just the lungs. ~15% are outside lungs, and ~1-2% are ocular. There, it can cause a diverse range of symptoms but frequently presents as uveitis (the uvea being the middle eye). If uveitis worsens despite glucocorticoid therapy, TB should be considered.

FYI UHC is the bane of healthcare providers' existences. They drove the push for HMOs with legislation in the 1970s requiring everyone be offered an HMO plan. They have a vertical monopoly with Optum. They deny treatments simply because automatic denials = more money, not on any medical basis.

The more I research Droperidol, the more I'm convinced that it was "killed" by insiders at the FDA to benefit big pharm. Heavily in use for 30 years with few serious complications (almost all were in other countries where 10-100x megadoses were given), then killed overnight with a blackbox warning.

I've started to think about this like the dark pattern f2p videogame ads you see everywhere. If an {ad,bot} interacts with 1000 people, and 10 have a response other than to ignore it, and 1 of those becomes a {whale,"cow"} sucked into a loop/cycle to produce {money,torment}, it's profitable to them.

We've seen a big uptick in the states as well. Rates of childhood scurvy tripled between 2016 and 2020, with 65% of children having an autism diagnosis (and thus a restrictive diet). I need to remember vitamin C deficiency for differential diagnoses for weakness with anemia.

A few years back I was at the SJ beach boardwalk and the lady's room had a giant line, so I put on a layer and used the men's. "I don't pass that well, it's fine." As soon as I was in a stall, I hear "did you see that? There's a GIRL in the MENS ROOM." I finished very quickly and got out asap ☠️

Had my first street medicine shift today. Hopped in a van with a couple other medical professionals and a social worker and met with a few patients at low income housing / homeless encampments. It's really nice to get to spend like 45 minutes with a patient instead of 3.
