Delighted to see our Decaf by Default work published in BMJ Open Quality. Offering decaffeinated drinks by default, while preserving choice, was associated with fewer falls and a better ward environment. Now Trust-wide. 👉 Open access: bmjopenquality.bmj.com/content/15/3...
When we serve you hot drinks on our wards, they will be decaf by default.

You will still be able to request a drink with caffeine, but our norm is decaf (decaffeinated) coffee or tea.

We adopted our approach after pilots on some of our wards. These showed how the switch to decaf reduced falls while patients try to go to the toilet.

Other benefits included:

Improvements to skin health
Calmer patients and reduced agitation
Better sleep
Why?

Caffeine is a natural chemical with stimulant effects. This means it increases activity in your brain and nervous system. It is found in tea and coffee as well as many other products.

Decaf tea or coffee is where the caffeine is removed from the coffee beans or tea leaves. It may still contain very low levels depending on how it was produced.

Having an overactive bladder means that people may experience:

Urgency
Frequent need to go
Waking during the night to wee
Caffeine can make this worse as it can irritate the bladder.

By reducing that sudden urge to wee, it helps reduce the risk of falling. This can happen when rushing to reach the toilet in time.

Other benefits of switching to decaffeinated tea or coffee include:

Improved sleep
Reducing noticeable / irregular heartbeats (palpitations)
Helping prevent dehydration and headaches
Reducing indigestion
Reduced risk of a condition that weakens bones (osteoporosis), as high levels of caffeine prevents absorption of calcium

Families sometimes report feeling guilty when their loved one has #delirium but it has been missed. The truth is that most members of the public haven't been informed about what delirium is or how to spot it. We need a public health awareness campaign on delirium.

But something else happens too. All the time I was there, my neighbours on the ward were two older women who weren’t entirely sure where they were and one of whom very much didn’t want to be wherever it was. They both had chronic conditions. They were incontinent. They were uncooperative. But every hour of every day there were one or two nurses or healthcare assistants or carers who made time to answer their calls, to keep them safe and clean and warm and sometimes even to get a smile out of them.

If you haven’t experienced it, it is hard to explain the effect of witnessing this level not only of professionalism and patience, but of love. There are studies that show that for all the criticism and brickbats aimed at the NHS, those who use it most are the most positive about it. When you’re there and watch, you understand.
