I had an operation last year and am still receiving care as on a waiting list for a second operation (nothing too serious) . I have had appointments at 3 different hospitals . One is in my home town and is walkable. The other 2 are really only accessible by car from where I live as public transport

The main answer is separate infrastructure. But it's also mutual respect - cyclists should give priority to pedestrians; pedestrians should recognise that cyclists passing them with 1.5 metres of space didn't "nearly hit them" even if they didn't hear their bell & that both deserve to travel safely.
