The fact I want everyone to know this World ME Day is ....

Fact: ME is an incapacitating illness. "No-one wants to be ill with this. It has robbed us of careers, income, relationships, becoming parents, having a spouse; being able to spontaneously go on a trip, to a concert, show, hike, dance, or whatever other enjoyable thing used to do. We live small limited lives in our h and for some confined to bed. We all exist with clipped wings, wishing only to be free.

PLRF Event Highlights Promising ME/CFS & Long Covid Biomedical Research Projects
July 24, 2025
8:24 pm

On July 9, the Patient-Led Research Collaborative (PLRC) hosted an online presentation by researchers funded by the inaugural Patient-led Research Fund (PLRF). Ms. Gina Assaf, one of the PLRC founders, started the afternoon by reviewing how the PLRC began and the PLRC’s overarching goals.

In April 2020, Gina explained, a group of people from countries all around the world and from diverse professional backgrounds banded together to document their experiences having Long COVID. This informal group grew into the PLRC, whose mission is to represent people with Long COVID and other infection-associated chronic conditions through research in which patients help conduct, through medical education, and through advocacy grounded in patients’ firsthand experiences. Even today, each PLRC team member is a patient with Long COVID or Long COVID–related condition.

Interesting new research from Spain: Metabolic adaptation and fragility in healthy 3-D in vitro skeletal muscle tissues exposed to Chronic Fatigue Syndrome and Long COVID-19 sera. iopscience.iop.org/article/10.1... #LongCovid #PASC #postcovid #postcovid19 #MEcfs #CFS #PwME
Abstract
Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) and Long COVID-19 (LC-19) are complex conditions with no diagnostic markers or consensus on disease progression. Despite extensive research, no in vitro model exists to study skeletal muscle wasting, peripheral fatigue, or potential therapies. We developed 3D in vitro skeletal muscle tissues to map muscle adaptations to patient sera over time. Short exposures (48 hours) to patient sera led to a significant reduction in muscle contractile strength. Transcriptomic analysis revealed the upregulation of glycolytic enzymes, disturbances in calcium homeostasis, hypertrophy, and mitochondrial hyperfusion. Structural analyses confirmed myotube hypertrophy and elevated mitochondrial oxygen consumption in ME/CFS. While muscles initially adapted by increasing glycolysis, prolonged exposure (96–144 hours) caused muscle fragility and fatigue, with mitochondria fragmenting into a toroidal conformation. We propose that skeletal muscle tissue in ME/CFS and Long COVID-19 progresses through a hypermetabolic state, leading to severe muscular and mitochondrial deterioration. This is the first study to suggest such transient metabolic adaptation.

A black and white image. The author of the post is laying in bed, he’s a man in his 30s. He has a beard and a moustache. You can see his head and part of the shoulders. His head is turned sideways and his looking at the distance. The picture is dark, the harsh light is hitting his head and lighting his facial features, like his Roman nose. He is positioned in the left side of the photo and is looking at that direction. Behind his head is a lot of negative space, there is a part of his pillow, that is under is head, and part of the bed is shown.

Highlights
•
Persistent cognitive fatigue is common in individuals with Post-Acute Sequelae of SARS-CoV-2 Infection.
•
We explored brain activation in PASC patients during a fatiguing cognitive task.
•
PASC patients showed differences in brain activation in fatigue-related and additional regions.
•
PASC patients also had increased frontal task-related activation compared to controls.
•
Findings suggest PASC patients may exert more mental effort during demanding tasks.

Interesting hypothesis with data in mouse models but a long way to go before human clinical application. IL-6 has paradoxical effects on skeletal muscle based on concentration and mechanism of receptor signaling, and the brain is far from the only source because circulating immune cells make it too.

Please share widely! This was the best an inquest could do. It took 4 years and the death of my only child to get here. NHS England must keep to this promise; the simplest change to a long established narrative that killed Maeve and threatens every other person with #ME. 🙏 @gwynnemp.bsky.social
