Several of my classmates in medical school were graduates of military academies like West Point, where they took classes in organic chemistry just like the rest of us. Does it really help the military to hobble these schools’ ability to recruit top orgo professors?

In a trial of a blood-based multicancer early-detection test added to usual care, the incidence of stage III or IV cancer after three rounds of screening did not differ significantly from that with usual care alone. Full NHS-Galleri trial results: nej.md/4xRZN5L
A bar chart from an Original Article published in the New England Journal of Medicine article titled "Effect of Screening with Multicancer Early-Detection Test on Late-Stage Cancer Diagnosis." The chart shows the distribution of the route to cancer diagnosis according to trial group after three annual screening rounds. The trial name, NHS-Galleri, is in parentheses after the title. The NEJM identity sits at the bottom.

There’s a lot of doom & gloom around how AI is affecting higher education. To a certain extent, I get it. AI is making some things harder. But I’ve found that AI has in some ways greatly enhanced my teaching. For one, it’s opened up applied activities that weren’t possible before. One example: 🧵

Think about how killer viruses, bacteria, and animals developed their deadly traits-- evolution by random genetic mutation and natural selection, sometimes over billions of years. It's frightening to consider an entity that can edit its own code quickly, at a massive scale, toward its own goals.

Disentangling Sources of Variation in C-Section Rates
(Forthcoming Article) - Cesarean section rates vary widely across U.S. counties, but whether this variation reflects patients’ medical risk, beliefs, and preferences or obstetric care environments, including clinicians’ practice styles and delivery facilities, remains unclear. Using 1989–2019 U.S. birth records, we study closures of counties’ last operating obstetric units, which redirect mothers to counties with different baseline C-section rates. In an instrumental variables framework, we find that a one-percentage-point increase in the delivery county’s C-section rate raises a mother’s C-section probability by approximately one percentage point, highlighting the central role of where mothers receive obstetric care in determining C-section use.

Clause and Effect: Theory and Field Experimental Evidence on Noncompete Clauses
Bo Cowgill , Brandon Freiberg , Evan Starr

Abstract
We study worker noncompete clauses in a large field experiment with two finance firms. Across ~14,000 job offers to freelance recruiters on short-term contracts, we randomize wages and the presence, salience, and duration of noncompetes (all contracts also included a nondisclosure agreement). Removing a noncompete increases mobility between competing employers by 36–52% and raises workers’ total earnings from the two firms by 12–17%. We find no evidence—rejecting even small effects—that removing noncompetes generates secret leakage. We also find no evidence that workers choose noncompete jobs for higher pay. Many workers appear unaware of noncompetes before firms’ post-employment communication. The results align with a model of inattention and uncertainty about enforcement.

Forthcoming in AEJ: Economic Policy: "Health Insurance and Access to Care for the Near Elderly" by Michael Anderson, Carlos Dobkin, Nicole Maestas, and Liam Rose.
aeaweb.org
Health Insurance and Access to Care for the Near Elderly
(Forthcoming Article) - The vast majority of Americans become eligible for Medicare at age 65. This results in 7 percent of the near elderly transitioning from uninsurance to insurance and 60 percent switching from some source of insurance to Medicare. We develop an extension of two-sample IV to estimate the effect of uninsurance on access to care while accounting for the effects of the other transitions. We find that not having insurance results in much lower probabilities of having a medical provider or receiving inpatient care. The foregone hospitalizations are for life-threatening emergent conditions and elective surgeries that increase quality of life.
