Antimicrobial resistance (AMR) is one of the major global public health threats. In recent decades, Acinetobacter baumannii has become one of the most significant multidrug-resistant (MDR) bacteria in hospital settings (1, 2). Since 2017, the World Health Organization has classified carbapenem-resistant A. baumannii as a critical-priority pathogen, a status that remains in the most recent update in 2024, which reflects the urgent need for new antibiotics against this bacterium (3, 4). A. baumannii is a frequent cause of nosocomial infections, primarily affecting patients in intensive care units (ICUs), immunocompromised individuals, patients using medical devices (such as catheters), and those with underlying diseases (5).

Antibodies targeting phage fiber and nozzle proteins impair Acinetobacter baumannii phage therapy by blocking infection and promoting immune clearance
by Heng Xue, Xinfeng Li, Guibo Rao, Fen Hu, Mingyue Zhong, Paulina Miernikiewicz, Rui Qiu, Sheng Cao, Krystyna Dąbrowska, Hang Yang Phage therapy represents a promising strategy to tackle the growing threat of antimicrobial resistance. Increasing evidence has demonstrated that phage-specific antibodies may compromise the efficacy of phage therapy. Nevertheless, little is known about how phage protein specificity of antibodies modulates phage therapeutic outcomes. Herein, we utilized AbP20, a podovirus against Acinetobacter baumannii, to explore the effects of antibodies elicited by each structural protein on phage therapy efficacy in a mouse infection model. Intraperitoneal administration of AbP20 once a day for 7 consecutive days induced robust phage-specific antibody responses that impaired phage therapy. Genome-guided antigen screening identified that antibodies elicited by the nozzle and fiber proteins of AbP20, rather than those targeting the portal, capsid, or adaptor proteins, are the dominant drivers of phage therapy failure. Specifically, fiber-specific and nozzle-specific antibodies block bacterial adsorption and genomic injection of AbP20, respectively. Meanwhile, both antibody subsets efficiently induce large phage aggregates and potentiate macrophage phagocytosis via an Fc receptor-independent pathway. An evolved AbP20 variant with improved neutralization escape capacity exhibited comparable antibody-accelerated phagocytosis and host immune clearance, yet partially rescued therapeutic failure caused by neutralizing antibodies. Collectively, this study elucidates that nozzle- and fiber-elicited neutralizing antibodies impair phage therapy via dual synergistic mechanisms, which offers novel insights into the multifaceted modulation of phage-specific antibodies against phage therapeutic efficacy.

Infection prevention and control strategies for multidrug-resistant Gram-negative bacteria in healthcare: a narrative review
AbstractAntimicrobial-resistant Gram-negative bacilli (AMR GNB) are associated with poor patient outcomes, are increasing in prevalence and are recognized by national and international public health authorities as a major threat to human health. The most common resistance elements vary by organism, but resistant Enterobacterales, Pseudomonas and Acinetobacter species have reservoirs in the human colon, natural environment and healthcare settings. Persistence of these organisms on inanimate surfaces contributes to the risk of bacterial transmission in healthcare settings. Standard infection prevention practices including hand hygiene, low-level disinfection and use of isolation precautions are effective in interrupting transmission of AMR GNB. However, the optimal use of isolation precautions is an area of active reconsideration and research. High-risk equipment, such as duodenoscopes, can be difficult to disinfect and has been associated with hospital transmission. The immunocompromised and critical care populations are at higher risk of infection and may benefit from additional infection control interventions. Innovative approaches to reduce healthcare transmission have included screening surveillance with the use of isolation or decolonization protocols, but these approaches require further study. Antimicrobial stewardship remains a key strategy to reduce the pressure to select for AMR GNB in healthcare settings and the general environment. Low-resource settings can implement existing World Health Organization infection prevention tools to reduce transmission of AMR GNB infections. Although implementation of existing infection prevention and antimicrobial stewardship strategies can limit transmission of AMR GNB infections, development of new approaches is needed to curb increasing prevalence of AMR GNB.
