30/43 Back to the critiques. We also name some names—calling out dominant paradigms that are conducive to bad science (looking at you, components model of addiction).
Given the above context, in attempting to define what constitutes a behavioral addiction, researchers in this domain must often wrestle with the amorphous nature of addiction itself. Unfortunately, in many cases, this conceptual struggle has resulted in the arbitrary application of substance use disorder criteria and models to behavioral addictions, a practice that we contend is both shortsighted and misguided (Borgogna et al. 2025b; Grubbs & Boness 2025). Evidence of this pitfall can be found, for example, in the uncritical and widespread use of the Components Model of addiction that is applied liberally within behavioral addiction research (Griffiths, 2005). This model, in its simplest form, argues that addictive disorders share six core components (Salience, Mood Modification, Tolerance, Withdrawal Symptoms, Conflict, and Relapse). This framework is highly influential within the behavioral addiction literature and frequently invoked as a rationale for conceptualizing diverse behaviors as addictions based on their apparent resemblance to substance use disorders and other behavioral addictions. Yet, the validity of the model is contentiously debated (Fournier et al. 2023); and modern conceptions of addiction clearly refute the notion that either tolerance or withdrawal are necessary or sufficient conditions in diagnosing addictive disorders (Hasin et al. 2013). Similarly, the mood modification aspects of excessive sexual behaviors were explicitly excluded from the ICD-11 criteria for CSBD (Kraus et al. 2018), and evaluations of their value in diagnosing CSBD have largely rejected them (Grubbs et al. 2023b). Collectively, even these simple examples demonstrate how influential frameworks can become reified within a literature, leading researchers to prioritize demonstrating similarities to substance use disorders over establishing whether a proposed condition represents a distinct and clinically meaningful disorder.

18/43 The International Sex Survey is a great example (40+ countries and 26 languages!), but far from the only one. Global collaborations are basically the norm here, which lets us do amazing cross-cultural work all the time.
Taking CSBD as an example, over the course of 2021-2023, a 42-country, 26-language research collaboration titled the International Sex Survey was conducted (Bőthe et al. 2021). This survey was led by a diverse team of international researchers and resulted in a final sample of over 82,000 participants. The results of this initiative were staggering, with over 30 publications arising from this study to date. Importantly, a key goal of this collaboration was the establishment of uniform, internationally validated assessment protocols across languages and cultural contexts (Bőthe et al. 2023), resulting in 19 papers alone dealing with the cross-cultural validity of various measures relevant to CSBD. Moreover, even among scales not included in these large-scale cross-country efforts, cross-cultural validation is common (Grubbs et al. 2023b).

10/43 Behavioral addictions aren't new. We trace discussions of behavioral addictions back at least a few millennia... and, arguably, they've been recognized for as long as psychiatry has existed
Humans are prone to excess. Stories of excessive substance use exist in historical records in virtually all cultures dating back thousands of years. Yet, there is also evidence throughout human history that behaviors themselves, not just substances, can become excessive or out-of-control. From ancient philosophers to modern smartphone applications, there is a millennia-long tradition of people attempting to change, reduce, or eliminate behaviors that they deem inappropriate, immoral, or excessive. For example, excessive gambling was first written about as a mental health concern as early as 1561 in the French philosopher Pascasius Justus Turq’s treatise On Gambling, or the Remedy to the Desire of Gambling for Money, which described excessive gambling as a disease (Nadeau & Valleur 2023). Furthermore, the struggles of the person vexed by excessive gambling were immortalized in Dostoevsky’s 1866 novel, The Gambler (Dostoevsky 1866). Despite this history, psychopathology-related fields have been slow to recognize such behavioral patterns as discrete mental health disorders. In the case of gambling, official recognition as a psychiatric disorder only came in 1977 with the inclusion of Pathological Gambling in the ICD-9 (Fauth-Bühler et al. 2017).

1/43 Ok folks, after much teasing and anticipation, I'm THRILLED to share our new paper in Annual Review of Clinical Psychology. Not gonna lie: I'm proud of this one, and I hope it makes an impact on the field (so please share!). A thread 🧵👇
To Be a Mental Disorder: Revisiting Behavioral Addictions
Joshua B. Grubbs, * Nicholas C. Borgogna, Beáta Bőthe, Hyoun S. Kim, Rory A. Pfund, Shane W. Kraus

Manuscript to appear in the 2027 edition of Annual Review of Clinical Psychology, this preprint may not
exactly represent the final printed copy.

Grubbs, J.B., Borgogna, N.C., Bőthe, Beáta, Kim, H.S., Pfund, R.A., & Kraus, S.W. (2027) To Be a Mental Disorder: Revisiting Behavioral Addictions. Annual Review of Clinical Psychology. 10.1146/annurev-clinpsy-061225-045608

Behavioral addictions have been recognized in various forms for centuries, yet only three—Gambling Disorder, Gaming Disorder, and Compulsive Sexual Behavior Disorder—are currently included in formal diagnostic systems. In the past decade, research on these conditions has expanded dramatically in both volume and methodological rigor, drawing on longitudinal, neurobiological, and cross-cultural designs and evolving into a truly global science. Even so, the field remains beset by unresolved controversies. Chief among these is a persistent tendency to import substance use disorder concepts—tolerance, withdrawal, and dependence—that lack conceptual and empirical support when applied to behavior. Related challenges concern the over-pathologizing of normal engagement and a reliance on confirmatory, self-report measurement of uncertain validity. We argue that advancing the field requires sustained funding, theory grounded in the disorders themselves, objective and intensive-longitudinal methods, greater diversity, and public health and regulatory attention to increasingly technological environments.

A 3,400 year old Ancient Egyptian ‘Senet’ game board with gaming pieces. Senet is one of the earliest known board games. Popular in ancient Egypt, it was associated with the soul’s journey through the underworld and was often placed in tombs. Brooklyn Museum 📷 by me #Archaeology
My photo shows an ancient Egyptian Senet gaming board. It is a small rectangular box made of glossy bright blue faience with a separate sliding drawer at one end (shown at left) for storing the blue faience gaming pieces. Six gaming pieces are shown on the game board on top of the box. Three are reel-shaped and three are semi-conical. There are four reel-shaped pieces and one semi-conical piece on the display case surface beside the game board. The long side of the gaming board is decorated with alternating ‘Isis Knot’ and ‘djed pillar’ symbols. The gaming board dimensions are 5.5 x 7.7 x 21 cm. On display at Brooklyn Museum. Dated New Kingdom, Dynasty 18, reign of Amenhotep III c. 1390-1353 BC

Here’s the Brooklyn Museum Catalogue description:

“Top of board divided into thirty compartments with dividing lines glazed black. Four squares inscribed with numerals and a nefer sign. Long sides decorated with painted design of seven panels, each with alternating ankh and djed signs. Solid end bears painted Horus name (Amenhotep III?) ‘beloved of Amon.’ Open end pierced twice on one side. Base glazed black. Ends of drawer glazed dark and light blue in checkerboard pattern. Base and interior of drawer glazed dark purple-blue. Front of drawer pierced with four small holes (for handle?); sides pierced three times and two times - purpose not evident. Condition: Object has been assembled from fragments. Extensive scattered areas of plaster restoration. The drawer was probably originally much longer. Glaze worn in spots. Numerous firing cracks”.

The Trier Gold Hoard! The largest #Roman gold coin hoard ever found! More than 2,650 Roman aurei, weighing 18.5 kg, were discovered inside a bronze vessel wrapped in leather bags, during construction work in 1993. The coins date from 63 AD to 196 AD. Rheinisches Landesmuseum Trier 📷 by me
My photo shows the so-called ‘Trier Gold Treasure’ in a museum display case. It is a Roman coin hoard made up of some 2,650 gold aurei. The small round gold coins are spread randomly in a dense pile across a light-coloured display case surface. To the upper right of the coin mound is the broken bowl-shaped bottom of the original copper alloy vessel that held the hoard. The metal is coloured green with corrosion. Several gold coins are scattered inside the bowl. In the lower left corner of the display case is another large, irregular fragment of the original container.

Museum information label:

In 1993, a bronze vessel with 2650 Roman gold coins (aurei) is discovered in Trier. It is the largest Roman gold coin treasure ever found. The Aureus was in the 1st and 2nd Century the standard coin of the Roman gold minting with an average weight of 7.27 g, with a very high fineness of approx. 980/1000. The coins depict 29 different emperors, empresses or relatives of the imperial house. The oldest coins were minted between 63 and 64 AD, the youngest between 193 and 196 AD. The coins were inside the vessel, which was accidentally discovered by an excavator, rolled up in leather bags. The bags were decorated with leather straps and closed enamel seal capsules.

The treasure revealed numerous secrets in its scientific processing: it probably did not represent private assets, but a state treasury that was carefully managed and over a longer period of time and enlarged. During a civil war, the gold coins were finally buried in a cellar in 196 AD and then fell into oblivion. Presumably the former administrator of the treasury took his knowledge of the hiding place with him to the grave.
