Are there good data on bone penetration for MDR TB drugs, incl the new ones? Particularly interested in penetration into bones with poor blood supply. Presumably quinolones and LZD get in okay. What about the other drugs? #TBsky

I find the argument made here problematic. Either it is ethical to randomise to a placebo or it isn't. I'd do response adaptive randomisation.

I wouldn't fund a stand alone RCT but I could perhaps support a Vit D randomisation in a TB prevention platform trial

I can see where you are coming from, but the Vit D - TB trials were small compared with other prevention trials Big savings can be made elsewhere, e.g. trial regulation, affordable placebos, trials embedded in routine care, platform designs, fewer observational cohorts on questions amenable to RCT

At NNT 1000, you could prevent 60,000 cases of TB per year in South Africa ~ 720,000 DALYs South Africa is willing to pay USD 3200 per DALY, so USD 2.3bn approx threshold for this intervention I suspect you could fortify commonly eaten foods in South Africa for much less than that?

For individual patient treatment, I would offer something low risk with NNT of 100, perhaps 200, to prevent a case of TB For mass supplementation, I would be interested in smaller effects

The 1.1 is for an infection endpoint, and the negative South African trial also had an infection endpoint There is no good reason to think Vit D would reduce Mtb exposure Primary disease endpoints were 0.87 (0.49-1.55) in Mongolia and 0·78 (0·54-1·13) in Tanzania

I'm not convinced that the TB trial is negative, I think they chose the wrong primary outcome I think the big scandal here is how much it costs to run RCTs of low risk interventions

Yep, its never been based on contribution. Why should a newly arrived migrant entering the workforce be treated any differently to e.g. an 18 year old entering the workforce? Make a decision at the border, then treat everybody in the country with dignity and compassion.

Sounds like a similar scenario. People on Health and Care Visas have access to NHS but are not able to access lots of community services, support with housing, social security For example, I've had patients I was unable to discharge from hospital because they were not eligible for community rehab

At the moment, people on Health & Care Worker Visa have no recourse to public funds. Why deny social security if they e.g. injure themselves at work? Treating carers with respect should include looking after them if they fall on hard times. Current rules deeply exploitative @stellacreasy.bsky.social

I think no unmeasured confounding is always going to be tricky in observational data on treatment initiation in acute settings - e.g. 'patient looks awful' is often not fully captured by a set of numbers

In the Stata package metan, there is a model(IVhet) option If you prefer to stick with RE, there are alternatives to DL which offer more conservative coverage and, in this example, might appropriately have resulted in a confidence interval that did not exclude the possibility of no effect
