Among critically ill adults with metabolic acidosis receiving vasopressors in the ICU, sodium bicarbonate did not lead to a lower risk of major adverse kidney events within 30 days than placebo. Full SODa-BIC trial results: nej.md/4ofXcyq #CCR26 | @criticalcarereviews.com
This image is a graph from a medical study titled "Sodium Bicarbonate for Critically Ill Adults with Metabolic Acidosis and Shock" published in the New England Journal of Medicine. The trial name (SODa-BIC) is in parentheses next to the title. The graph shows survival probability over 90 days for two groups: one receiving sodium bicarbonate and the other a placebo. The horizontal axis represents days since randomization, while the vertical axis represents the probability of survival, ranging from 0 to 1. A legend indicates the number of patients at risk over time for both groups.

I'll have A LOT of retatrutide thoughts. Alot. But these cardiometabolic improvements are stunning. 40% drop in triglycerides, 20% drop in LDL. 63% drop in hsCRP, 12 point reduction in BP. It's not a stretch to imagine this drug massively reducing death from CVD and CKD all on its own.

17 days until retatrutide phase 3 data and here's some published and unpublished data on decreases in glucose, insulin resistance and fasting insulin levels in diabetics and non-diabetics. This just in obese patients and the decrease in fasting insulin is *enormous* over 50% and still dropping!

Generic Farxiga/Metformin combo pills are now starting to be dirt cheap locally with GoodRx!! By year's end I expect my new diabetic starter meds to mostly be these two. Especially if they have hypertension or high cholesterol or even slightly diminished renal or cardiac function. Yay for generics!
