If someone says they are trans, they're trans. Not everyone can dress as their preferred gender, not everyone can use their preferred pronouns, and there's a whole multitude of reasons someone might not have access to or a desire for medication or medical procedures. I don't think it's complicated.

No judgement, I've been there, I hold myself back from doing this. But if the Dawnwell didn't happen, and Zul'jan became a heroic character and Vol'jin returned with the whole Horde, you would've forgiven if they gave dog poop in the monthly reward and added a 500$ mount in the store. It's normal.
