I didn’t know rape is 4x more likely to result in diagnosable PTSD than combat, yet it makes sense. The diagnosis is related to overwhelmed coping resources, a sense of powerlessness, and often confusion or self-blame about what happened. That’s where the brain gets stuck.

There is a moment at the very very end of Abdul El-Sayed's podcast with Ezra Klein (book recommendation section) where he talks about the idea of "speaking truth to pain" and this will be staying with me for an extremely long time. www.nytimes.com/2026/09/25/o...
But there’s also something about truth to pain. We talk a lot about truth to power, but I think it’s actually a lot harder to speak truth to pain. I say that as someone who went to medical school, realizing that in a lot of our politics right now, we’ve got to get a lot better at speaking truth to pain if we’re going to get through some of these challenges that we have.

What does “truth to pain” mean?

It’s one thing to look power in the eye and say: You’re wrong about this. But there’s a way in which pain is very difficult to reason with. Because when somebody is in their pain, they’re in their trauma. They see that pain, and they may not see what’s causing it.

I remember just very explicitly being in a trauma ward with a gunshot wound patient and watching this big guy with his hand on his gunshot wound. All the folks are trying to pull his hand off his pain, that point of his gunshot wound, this clear vulnerability.

And this emergency room doctor had the presence of mind to be like: Everyone, stop. And she looks him in the eyes, and she’s like: I get that really hurts, and I’m so sorry that you’re here. You don’t want to be here. We don’t want you to be here. But if you don’t let us fix this, you could die. So I’m going to take your hand, and I’m going to move it off, OK? Because I need to get there so that you can live.

She was speaking truth to pain. He understands he’s at the hospital, he needs care, but he also doesn’t want anybody to touch the wound. Which is a natural thing. And I think it is a really great metaphor for how we think about a lot of these challenges we talked about today.

Arguably it was Dan Turner’s obliviousness plus the judge’s male sympathy that made this case famous, when contrasted with Chanel Miller’s brilliance and courage. This sloppy letter helped make the son a public outcast for life. If he’d served even 6 months, he could have stayed unknown.

What Being a Good Guy isn’t: Saying nothing to shut down horrible behavior in the moment, then telling the victim “that wasn’t okay.” Or recounting to your woman friends afterwards that you noticed how fucked up it was. Sit on any bus, and you’ll see older women step in where grown men won’t.

It is good at expressing uncertainty numerically while being remarkably bad at sustaining deeper forms of doubt about the conceptual machinery within which those numbers are generated.
Probabilities, confidence intervals, Bayesian updates and elaborate forecasts can produce an impressive surface of epistemic modesty while leaving more fundamental premises remarkably insulated: what counts as value, which futures matter, which problems deserve priority, which forms of evidence are legible, and who is entitled to perform the prioritization.

But I just did what nearly all writing about the issue does: talked about the percent of women on campus who will become victims without talking about the percent of men on campus who will become perpetrators. Most campus rape victims know their assailants.
1 in 8 students experiences sexual violence during college.
The 2019 Campus Climate Survey found that 13% of undergraduate students experienced rape or sexual assault after enrolling in college.

That’s about:

• 1 in 4 undergrad women (26%)

• 1 in 14 undergrad men (7%)

who experienced nonconsensual sexual contact by physical force or inability to consent since enrolling at their school.
