Yeah 2 HIV-2 viral tests neg over 6 months. I am inclined to say this is false positive too! Tried to look for schisto/strongy as i saw some literature for them to cause cross reacting ab. Both negative.

Foscarnet/cidofivir are options too but they carry their own risks. I feel more comfortable with gan/valgan. Intravitreal is an adjuctive option as @boghuma.bsky.social suggested.

Personally, would do Ganciclovir/valgcv + gscf support intermittently to keep ANC >1000 if the issue is only agranulocytosis! Would switch PCP SXT proph to SS/day or switch to Dapsone to minimise other drug contributions to agranulocytosis.

Personally I would continue ART- no harm in taking this with GI bleeding. Biktarvy can be dissolved and taken with a small amount of water if needed. The gut produces so much fluid, this would be inconsequential in my opinion.
