Showed the abstract to my wife, who rolled her eyes so hard at the idea we needed a trial to find out we should give more pain control when placing #IUDs. She was even more shocked to see this was only just published in #JAMA. jamanetwork.com/journals/jam...

Intrauterine Mepivacaine Instillation vs Placebo for Pain During IUD Placement
This randomized clinical trial assesses whether intrauterine instillation of mepivacaine reduces pain and improved tolerability during intrauterine device placement in nulliparous individuals.

the fact that the NTSB is conducting a federal safety investigation of the helicopter crash, but nobody is going to do anything about the original deadly car crash, really shows just how few fucks our country gives about every day traffic violence that kills 40k people every year

I often refer people to #hospice (normal in #geriatrics). If Oz wanted people on hospice to get palliative chemo or dialysis he could change the Medicare rules. He could make hospice facilities a thing so family caregivers aren't so overworked! What on earth is he wasting time on?

Thank you, @uclahealth.org for sharing my work! It's exciting that potentially important data on health and immunity are hidden in a TB test we routinely order. I hope this opens up new ways to study the immune system at a population level. www.uclahealth.org/news/release...

Data hidden in tuberculosis screening tests sheds light on patients’ overall mortality

Lucky enough to get tickets to #LA28. The price gap between men's and women's Olympic #basketball events is a chasm. It's not much better for other sports.
Tickets to men's and women's Olympic basketball games. Prices for the women's bronze medal game start at $106, while men's bronze game starts at $508. The women's final starts at $636.

ALL drugs are potentially unsafe. A rule of thumb in #geriatrics is that a new patient problem is likely from a new medication. It could be an interaction, it could be issues with drug metabolism. Many problems are better-address without medications at all. Prescribing is key to patient safety.

Let it not be lost on us that the flu season is real, real bad the year that the administration fucked around with the flu vaccine committee and decided not to cooperate with international disease experts. Stark cause and effect, and it's only going to get worse if we let RFK continue his work.

With the changes to the #CDC child vaccine schedule, expect to see more older adults get seriously ill from preventable #flu, #RSV, and #COVID infections. Vaccines for children protect the people who look after them, too.

"Ryleigh did not receive a #flu shot this year." Horrible. Preventable. Transmissible. I take care of older people and it's awful enough when they get seriously ill and disabled or die from severe flu. I never want to know the heartache when it's a child. Get #vaccinated.

I may be preaching to the choir, but get your #flu shot! Scared of needles? There's an inhaled vaccine! Allergic to eggs? We've got recombinant and cell-based vaccines! Dislike the US #healthsystem? Any #vaccine at all will make you less likely to have to interact with it!

@rbreich.bsky.social's blog post today gives a clear example of #ageism. It's sadly persistent. This is mostly bias against older adults. For clinicians, this is also not knowing how to assess aging physiology. #Frailty provides some tools to help. Many are at efrailty.hsl.harvard.edu #medsky 1/8

Fair enough, so a few things about #shingles #vaccine side effects below. Caveat: my patients have largely already received it, so I haven't administered it in some time. This is also, of course, not medical advice, just FYI.

The evidence is also good that the #shingles vaccine prevents #dementia. That means you're less likely to need to see me, which is definitely a win for you. Anyone telling you not to get vaccinated wants to off you and possibly take your stuff.

I trained during some of the awful flu seasons before COVID. It wrecked hospitals: not enough beds, not enough staff (2/2 influx of patients & staff getting sick). This hurt not just people with flu but everyone who came in for any kind of medical care. Vaccination saves lives in more ways than one.

I really hate it when scientists keep saying that “we need to rebuild trust in science,” because it implies that scientists are to blame for the mistrust rather than the millions of dollars of dark money that have funded political attacks on science in order to advance a far right agenda.

This is awful science journalism, @mittechreviewbr.bsky.social. The tools in the article help assess pain in people who can't communicate (intra-op, ICU, dementia, neonates), but the author acts like it's for everyone. People then understandably treat this is dystopian rather than mundanely helpful.

I know this is nit-picky, but it's "cojones". Having drawers in your home doesn't mean you're able to stand up for democracy. Also, this very much is a popularity contest. If people hold their nose to vote for you, you're probably not going to get them to rally or take risks when it matters.
