Going to the gas station is probably one of the things I hate most. I love having an electric car! It is the best. And couldn’t come at a better a time. I saw gas prices at $5.33/gallon yesterday 😱. Congratulations on your new car!

Yeah, I agree. I have yet to use it but at least now I have a tad bit more understanding on when it may be important. But those circumstances are exceedingly rare in my patient population. I can think of 1 patient who it may be helpful for.

HIV proviral DNA testing has always been a bit of a mystery to me. It’s not easy to get in community clinical practice, and I am not always sure when it is important. This was a great review that helped summarize when this could be impactful to direct patient care.

💯 Also depends on how cx’s are drawn. I have seen that also when minimal volume collected in 2nd set. It’s the whole clinical picture. Persistent positivity is a clearly a bad sign, but having just 1/4 is just one piece of data that I don’t put too much stock into without the whole picture

1 major + 3 minor = definite IE This is a perfect example of how number of positive blood cx’s is just one piece of data and does nor disprove IE. I also think we get fixated on the word “vegetation”. But I have seen plenty of cases where the pt has already embolized the vegetation everywhere

To me, that’s high enough that I think the burden of proof is to prove that IE is not there. Whereas I think it was much lower for the other strep species. I think virulence and biofilm properties of psar would have me more concerned

I the calculation would change if the entire population had a prosthetic valve. I also think that certain infections are such high mortality and morbidity, the NPV has to be 95%+. I briefly glanced through the study but I think they said 1 in 80 chance of IE if 1/4 blood cx + for staph aureus.

I don’t necessarily think you have to have numerous positive blood cxs to have PVE. That’s worst case scenario. I think the key here is whether or not you have another clearly defined source. If the TEE showed something suspicious, but not clearly IE, I would repeat TEE in a week vs empiric tx.

This is why I am gravitating to Roon because it’s meant to be for medical professionals only and not the general public. It’s crazy to want to read about escmid or idweek and be bombarded by all the drama and the bots. Hopefully it will be international and broader based medical community soon.

I have started spending a lot more time at Roon. It’s in the early phases but there are some interesting concepts that are being built there. Like spaces for discussion, rounds, curbside consults. I just miss all the friends I made on Bluesky.
