Not that I’ve seen nationally. We have a paper from UCSF network hopefully coming soon… our local rates are up quite a bit but still low esp among younger men

Great Q. I’m optimistic. % of low risk disease at dx has fallen to ~20%, and rates of AS for low risk are rising. We’re increasingly able to define—and even implement—smarter screening.

Totally agree. NPV for MRI over many studies is consistently more like ~90% — when read by high volume experts. MRI pre-biopsy is super helpful, but for a reflex test to r/o bx altogether we have better options that are also (in the US and only in the US) a lot less expensive.

How often is AS/WW used in int risk prostate cancer? Leapman et al @yalecancer.bsky.social report in JAMA that AS/WW use increased from 5 - 12.3% from 2010-20, most in GG1 dz. Treat high-risk disease early, but offer AS/WW for low + some int risk cancers. jamanetwork.com/journals/jam...

Preacher -> choir! I’m still solidly in the camp of not even calling GG1 ‘cancer’ let alone treating it. But this is yet more support, imho, for early screening, _selective_ workup of ⬆️PSA, and tailored management of aggressive disease.

ICYMI: *30* year outcomes from SPCG-4 dx.doi.org/10.1056/NEJM... RP vs. WW for clinically detected #prostatecancer reduces CSM 48% and *ACM* by 26%. NNT down to 6. Clear benefit to treatment of clinically detectable disease if (and only if) your pt has a very long life expectancy. #pcsm

Um… in our benighted health care system liquid test is 1/3 cost of MRI & Ben knows I’m all about use the free info first and second! PSAD, GS 4 subtypes, etc. My fav thing about PI-CAI is exactly that it’s free :)

Ok I’m leaning RP too. Higher than usual risk of needing surgery for postop SUI… but if he already has a significant irritative LUTS he might wind up pretty miserable from radiation, which is harder to fix. If Decipher is 60s not 80s, cribriform is not expansile, PSAD is low, maybe close AS?

Thanks for adding one! Yes this one was terrific. Though given use of a reliable secondary test (MRI or otherwise) PSA threshold for evaluation should be much lower than 3, esp in midlife :)
