There was a big push to make this first line for a wider range of indications in my centre. I pushed back because we don't have fabulous cover out of hours in some of our peripheral hospitals and I think it needs to be used with nuance and support. So no change for me.

Our national guidelines weren't further updated with respect to this. It recommends for non severe hospitalised patients and outpatients at high risk.

Was it controversial? This type of study gets sent to me to justify the need for 48 hours abx. I know we're all trying to gain ground from different starting points. I do think language is important.

On #WorldHepatitisDay: Hepatitis E Virus causes 20 M infections/3.3 M symptomatic cases annually. A hepatitis E vaccine was 95.5% effective in a 2007 trial, yet was never developed further. Why? www.nejm.org/doi/full/10.... @worldhepalliance.bsky.social #idsky #episky #medsky #publichealth
Cumulative Hazard of Hepatitis E.
The cumulative hazard of a first hepatitis E episode (i.e., the incidence rate) was estimated as a minus-log transformation (log data to nonlog data) of the Kaplan–Meier survival curve during the period after the administration of the first dose of study drug until the end of the study among all subjects who received at least one dose of either vaccine or placebo. The numbers below the graph are the numbers of patients in each group remaining under surveillance at 100-day intervals. The difference between subjects who received placebo and those who received vaccine was significant (P<0.001 by the log-rank test). Vaccine efficacy from the first dose, calculated with the Cox regression model, was 89.9% (95% CI, 77.9 to 94.5%).

Generalisability will be heavily dependent on setting. Ceftriaxone or cefuroxime monotherapy would be optimistic in many settings in 2026. Curious about cluster design to answer this question too? Glad they reported it still as the as treated findings look concerning.
