Yes, exactly! In addition, immunocompromise itself may not be the risk factor we think it is. I’m thinking of the DRIP score and the new CAP paper in ASHE as examples.

I find it increasingly annoying when I hear “we need pseudomonas coverage b/c patient is immunocompromised from diabetes” regardless of infection or DM severity!

My first job after residency was on faculty at a college of pharmacy where I taught about fungal infections. It wasn’t until I moved to California’s Central Valley that I realized that was “the valley” in Valley Fever though 😆

And, look, I don’t need people to understand how they use their rights. I just need people to agree that the government shouldn’t be able to take any of our rights. I put my face on the ground thirty-four times a day, David. Most people don’t understand why I do it. I don’t need them to understand. I just want to have the right to do it. And when people decide that they want to take away my right to do that, I’ve got a problem.Now, I hope that everybody’s going to show up and say, No, no, no. You can’t stop that person or that community from reflecting and praying as they choose. Similarly, when it comes to trans rights, this is the right to get the health care that you need. That’s really what it comes down to. It’s the right to be who you are. It’s the right not to be discriminated against by your government because of who you are. We have some vancomycin doses in them. On the floor 1g and less is available to make at bedside with our adaptor system. The ED has some premixes. Vancomycin is easier than aminoglycosides since all those doses are rounded to 250mg.

Meropenem for all I think simplifies the issue too much. The goal is to have a regimen with the correct spectrum of activity and tissue penetration without being overly broad. Accomplishing this with something given less frequently, with a better safety profile, and improved logistics is the point.
