7/ This remains a hypothesis. But it turns an intriguing imaging finding into concrete, falsifiable questions about thalamic gain, bodily signal weighting and treatment response in ME/CFS. I thank the @sfb-trr-289.bsky.social for the support of this work!

6/ The key next step is to move beyond resting-state snapshots and track these processes within individuals after exertion, through the onset and peak of post-exertional malaise, and during recovery.

Andreas Strube @astrube.bsky.social · 29/07/2026
5/ This may provide one missing piece linking symptoms: sensory hypersensitivity, pain, orthostatic intolerance and cognitive “brain fog” could reflect a shared neurological gating disturbance across different domains.

Andreas Strube @astrube.bsky.social · 29/07/2026
4/ If thalamic filtering is altered, predictable ascending input may no longer be sufficiently attenuated. Signals that should remain in the background could instead dominate processing.

Andreas Strube @astrube.bsky.social · 29/07/2026
3/ The starting point is peripheral biology. Post-infectious immune, vascular, metabolic and autonomic disturbances can change the statistics of the body: the intensity, variability and reliability of the signals reaching the brain. The body the brain is inferring has changed.

Andreas Strube @astrube.bsky.social · 29/07/2026
2/ In this commentary, I argue that the thalamus may not simply relay bodily signals, but help determine how much they “count” by regulating their gain and precision.

5/6 Bayesian modeling supports a prior intensity shift instead of a precision modulation. Those who expect to wait longer shift their top-down prior toward higher intensity.
120
Andreas Strube @astrube.bsky.social · 14/04/2026
4/6 EEG reveals a clear dissociation: - Temporal expectations modulate α/β power during cue presentation (not during stimulation) - Prediction errors emerge in β/γ during stimulation - Actual stimulus latency is encoded in α/β frequencies
120
Andreas Strube @astrube.bsky.social · 14/04/2026
3/6 Key finding: Expecting longer delays → higher intensity ratings. Actual delays? No effect. Prediction errors? No effect on ratings. This holds for painful heat AND non-painful cold, which suggest not a pain-specific phenomenon but a general somatosensory mechanism.
120
Andreas Strube @astrube.bsky.social · 14/04/2026
2/6 The “dread effect” says that we prefer pain sooner rather than later. But is that driven by actually waiting longer, or by the expectation of waiting? Previous studies couldn’t disentangle the two.

1/6 Our paper is out today in Nature Communications! From the SFB/TRR 289 Treatment Expectation @sfb-trr-289.bsky.social we show that temporal expectations shape somatosensory perception - for both painful heat AND non-painful cold stimuli.
