Can you imagine being approved for a surgery, waiting years for the appointment, and when they day comes you finally meet the surgeon - it's cancelled by a subcommittee which has decided too many people in your country want the surgery.

A lot of people don’t realise how much that day hit trans people. That day the UK declared us unwelcome in our own country. That day we knew the years we spent jumping through hoops to do everything asked of us meant nothing. The day we began looking for an escape route in whatever form we can….

It will take a generation or more to undo what Labour have done. I turn 51 in a few weeks and I don't think I will see it reversed properly in my life time. Hope I'm wrong but I fully expect to die with fewer rights than every cis person I know.

Honestly, little has made me feel more depressed and hopeless in a while than seeing the way trans people are being spoken to by our "allies" of late. Difficult to describe just how bone tired I am. This work utterly drains you. Yet we are not even given the grace of being treated like real people.

I have asked the Chair of the Equality and Human Rights Commission to come to Brighton, and today Brighton came to me with so many stories of how the EHRC Code of Practice is impacting them. Trans lives are already being harmed, and the EHRC needs to hear this.

Important to remember today: If ANY Labour MP talks about “respecting” the Supreme Court misjudgement, they are *lying* to you. Parliament can overturn ANY SC decision & the government can amend the Equality Act to include trans people. The SC misjudgement still stands because Labour WANTS it to.

What evidence is there that patients were referred inappropriately?
At this stage, it is not known if patients have been referred inappropriately. The purpose of the Independent Clinical Review is to assess this.Have any patients undergone surgery inappropriately?
At this stage, the NWJCC has not identified any patients who have undergone surgery inappropriately.
However, there are cases where patients have been referred into the surgical pathway but have not gone on to have surgery, including where NHS England providers have identified that referral criteria had not been met. It is also recognised that concerns have been raised by some patients about the referral
process.
The purpose of the current review is to better understand these concerns and provide assurance that patients are being appropriately supported, assessed and referred, and that the necessary clinical governance arrangements are in place.Does this mean patients may have come to harm?
No immediate patient safety concerns have been identified by the NWJCC, and therefore they are not concluding that patients have come to harm.
However, concerns have been raised through complaints to the NWJCC, correspondence, and other information received about the gender affirming surgery pathway. As a precautionary measure, referrals, surgical assessments, and surgery are paused while further review work is undertaken.
These prompt actions have been taken to ensure any potential risks
are identified and addressed as quickly as possible, and that patients continue to receive safe and high-quality care.

Just 426 people were referred for gender-affirming surgery in Wales last year. This compares to: - 8,213 hip replacements - 7,851 knee surgeries - 25,633 cataracts surgeries Until NHS Wales publishes its reasoning, patients will be left in the dark about the future. It's time for answers.

Yeah, but the Government need to ask hard questions of the NWJCC. Given that JCC have admitted that at this stage there is no evidence that anyone has been referred inappropriately, and no evidence of any patient harm, why did they pull all surgeries? Just because Nick Wallis and pals told them to?

Organising - why bother? Lots of despair from trans folks on the tl and other places of late. Things are horrible and scary. I also think we’re doing brilliantly at fighting back given the asymmetry of arms at play. We are so deep in a pit at the moment in terms of trans rights in the UK.

Committee
We are writing to express our profound disappointment at the abrupt pausing of planned operations within the Welsh Gender Service. The sudden nature of this decision by the NHS Wales Joint Commissioning Committee has left patients, many of whom have been preparing for surgery for months or years, without the care they were expecting and relying upon.
The impact on individuals and the wider trans community in Wales has been significant.
Understandably, this development has caused distress, fear, and a deep sense of uncertainty.
This comes at a time when the rights and dignity of trans people across the UK are increasingly under pressure. You will know that gender-affirming healthcare in Wales is clinician-led, patient-focused, and carefully paced; it is not rushed, nor undertaken lightly.
In light of this, we would like to raise the following urgent questions on behalf of our constituents and the wider community:
• Why has surgery been halted for a form of treatment that has among the highest levels of psychiatric screening and one of the lowest regret rates of any procedure offered by the NHS?
• What specifically is the cause for concern?
• What evidence informed this decision?
• Why were patient stakeholders not consulted before the decision was taken?
• Was any risk assessment carried out regarding the impact of cancelling treatment at such short notice? If not, why?
• What role, if any, did NHS England play in influencing or directing this decision?
• What is the scope of any review now underway, and when can we expect its conclusions?Senedd Cymru

• What safeguards are in place to protect patients' mental health during this pause?
• What commitment can be given to the continuation of therapies and hormone treatment during this period?
Patients deserve clarity, reassurance and a clear pathway forward. We would be grateful for a detailed response addressing each of the above points, together with confirmation of how the Welsh Gender Service intends to support those affected during this period of uncertainty.
This letter is also being copied to Suzanne Rankin and Kirsty Williams, Chief Executive and Chair of Cardiff and Vale University Health Board, given the board's role in administering the Welsh Gender Service.

Interesting point here - even by the late 1990s, years before the GRA, the legal assumption is that a trans person will change the gendered facilities they use as soon as they "begin to present permanently in the sex to which they identify" No mention of medical transition or "sex"/"gender" dualism

The NHS Wales Commissioning Committee has forced the Welsh Gender Service to suspend all gender-affirming surgeries.

This nakedly cruel policy shift will wreak havoc on the lives of Welsh trans people. it will force patients into months of limbo that massively disrupt their lives, while NHS higher-ups take their sweet time to cook up yet another sham "investigation".

at worst, patients will be driven into worse mental health and suicidality. despite government cover-ups, we all know the true statistics about our community.The Welsh gender care system, while not perfect, is one of the best in the uk, and delivered for trans people by collaborating with the community and with GP surgeries, proving that positive change is possible.

This massive u-turn comes under pressure from the Welsh Tories, whipped into a frenzy by a terf blogger and Murdoch rag ‘The Times’, which claimed that wales having higher surgery referral rates than England somehow proves that harm is  done.

This is the most fucked up and immediately harmful policy decision I think I've ever seen in adult trans healthcare in the UK, and that's saying something. The Welsh Gender Service is one of the best in the country. It is widely praised by the patients it helps. That is why it is being targeted.

The Levy Review of England’s GICs concluded that waiting times were unacceptable, that this had serious knock on effects on patient welfare, and that the unnecessary waits for surgery also put unnecessary additional strain on the services (requiring repeated, redundant 6 monthly reviews)
33. Waiting times for these services are unacceptable.
Immediate steps must be taken, and all commissioned GDCs need to work as a national network to deliver patient pathway improvements.

Freelance journalist Nick Wallis has written an article on his Gender Blog after making a series of requests under the Freedom of Information Act.

He states: “Over the last three years the WGS has seen around 800 patients a year. The average age of those patients is 26. The crucial question is how many of those patients are recommended for cross-sex hormones after one or more appointments. Astonishingly, Cardiff and Vale University Health Board tells me ‘this information is not centrally recorded or collated’. The WGS doesn’t know what percentage of its intake it sends to the NHS’ Local Gender Teams (run by individual health boards) around Wales for endocrine assessment and hormone prescription.

“Cardiff and Vale University Health Board, which appears to be responsible for WGS, did give me some figures from March 2023 to Dec 2024, which suggested that throughout that period the monthly conversion rate from initial appointment to hormone plan varied between 51% and 88% of patients, though it did also say the data ‘was manually tracked and entered and so cannot be assumed to be 100% reliable’.
