we all have mental rolodexes of patients we never forget as an ER doc, I've taken care of quite a few patients where we found new -- almost always advanced -- pancreatic cancer this is great news and a nice reminder of why we all chip in and hire the best scientists to find new & better tools

How will you know without expertise? As we rush to deconstruct systems that have long functioned, we should be thoughtful about not causing unintended harms along with the significant potential benefits. Move fast and break things has its place, but in healthcare the "things" are humans.

ED shifts are also generally quite short, as ED care is so cognitively burdensome that most busy EDs have moved away from 10+ hour shifts, meaning patients undergo numerous signouts while boarding without an inpatient team, with attendant risks of information loss.

This paper draws attention to one such mechanism: patients who are admitted can not only wait for many hours (or days!) in some EDs waiting for beds, but in some institutions are left in the ED without their care being assigned to an inpatient medicine team.

6. When I was 24 years old, I wrote a long letter to my academic hero, John Maynard Smith, asking him for comments on a paper I'd written about one of his models. He had never heard of me but my missive launched a (pen-and-paper) correspondence that ran hundreds of pages and lasted until his death.

2. I guess this is the new form of academic engagement farming. Spin up some agents to search for relevant targets and then write each of them a letter, custom tailored to what the target has published, about whatever you’re trying to promote.
