Last June, I had a colonoscopy and the doctor found a polyp. He removed the polyp and, as is standard procedure, sent it to a lab for biopsy.
United Healthcare denied the biopsy claim because the doctor didn't get a seperate pre-approval for the biopsy.
This is insanity.
