Reducing a medicine can be the right decision but it often needs to be done very gradually. This is also something that needs to be taken into account when discussing deprescribing. 010

Of course we can't all be experts in everything, but I tell my students: wherever you work, you'll encounter people on these meds. 110

If you want to be super careful you could mark the packs to ensure you use them well in advance of the expiry. 130

I used to write the risk assessments in our hospital about what to do when ward stocks of medicines got too hot and for most stuff we basically had a formula for reducing the expiry date. 150

There are sometimes relatively simple fixes that could make things better if staff had time to pause and consider their service as a whole. But that's rare, and everyone loses as a result. 061
