Look out for crumbling Priadel tables reported by @luckyerica3.bsky.social You can safely halve Priadel but crumbling tablets could lead to lithium being released too quickly. Get advice from your prescriber or pharmacy. Avoid stopping suddenly. You can also report direct to the UK meds regulator.

[2/2] Two similar patients (on paper) could be on identical meds. After a medication review, one may choose to continue everything and the other may wish to stop one or more medicines. And both outcomes could represent good care. We need to be careful what we measure when we talk about #polypharmacy

I'm sorry this happened to your mum, I think you're absolutely right. Reducing a medicine can be the right decision but it often needs to be done very gradually. This is also something that needs to be taken into account when discussing deprescribing.

[2/2] There's a balance of risks and benefits which will be different for each person. There will be uncertainty about what will happen when a given med is stopped. Sometimes things get re-started, that's okay! These are complex consultations and we need continuity of care for them to be done well.

I think as Tim @acutemed2.bsky.social says, there is a need for individualised care which is frequently not quick and cheap to deliver. People need time to understand all the meds they're prescribed and support to make decisions about what they want to stop or continue [1/2]

I've loved the Lost Words Blessing for so long and today I got to sing it in a beautiful choral arrangement with a fab scratch choir in Bristol. Very excited to hear the whole Spell Songs ensemble later this week too.

I really enjoyed a visit to the London Fashion and Textile Museum with my teenager this week to see the exhibition of work by Susan Collier and Sarah Campbell. It was as joyful as the name suggests: Paint! Pattern! Print! 💕🌈🌼🦚🦋 The striped jug in the collage below was his artwork in response.

I had this audiobook version on cassette when I was a kid. My Dad had recommended it to me as one of the funniest books he'd ever read. I loved it, and could quote several passages verbatim for a time (no doubt to the annoyance of the rest of the family!)

My eldest (now almost 16yr) was a big fan of both Thomas the Tank Engine and Octonauts, and I recognise these thoughts! I do remember stumbling across this and similar pieces at the time. My academic background doesn't equip me to comment on its quality.

A professor of psychopharmacology showed us a set of Louis Wain's cats in a lecture many years ago and it had a profound effect on me, I think it was part of what led me into mental health work. But it's also been fascinating and instructive for me to later understand the myths about his work

I also think divisive views on whether mental health meds are "good" or "bad" can miss the point that regardless of the debates, lots of people take these meds and should be supported to start, stop or continue them as safely as possible with access to good information to inform their decisions.

I think a big part of the problem is the way that mental health meds get seen as so specialist that many working outside mental health kind of wash their hands of them. Of course we can't all be experts in everything, but I tell my students: wherever you work, you'll encounter people on these meds.

Unless it is something with v. specific storage requirements (as others have said: fridge items, or stuff that already has a short expiry like certain liquids) there shouldn't be an issue. If you want to be super careful you could mark the packs to ensure you use them well in advance of the expiry.

If it's any help, I agree that you don't need to worry about a few days exposure to higher temperatures. I used to write the risk assessments in our hospital about what to do when ward stocks of medicines got too hot and for most stuff we basically had a formula for reducing the expiry date.

This does seem to be a key barrier when people ask how can we get the best possible people in parliament. Being a candidate and then a backbench MP looks pretty awful to me. "Why We Get the Wrong Politicians" by Isabel Hardman really opened my eyes to this and changed how I think about politics.

There are *so* many examples where the onus is on vulnerable patients to navigate poorly functioning systems. There are sometimes relatively simple fixes that could make things better if staff had time to pause and consider their service as a whole. But that's rare, and everyone loses as a result.

We've worked hard to come up with ways to help him use platforms appropriately, with an awareness of the risks. We have frequent discussions about the time he spends online, we keep dialogue open. I know we will have to find our path through this, but right now I just feel anxious and sad.
