"Not dying at 0 hp" is probably the most common house rule used for people who otherwise are judicious adherents to B/X or OSE. What if every class had its own mini table of house rules of things that happened at 0 hp, though? What if you used MANY house rules for death?
Death
When you reach 0 hp, you may choose to die or roll on the following table.
Dismemberment. You take a grievous wound and fall unconscious. After a turn, you awake with 1 hp and learn that you've lost your 1. eye, 2. hand, 3. leg below the knee. You must retreat from the dungeon as soon as possible. You must rest for a week before adventuring again.
Taken. The monsters [take you prisoner/swallow you] and retreat. You may be [rescued/regurgitated] and restored to 1 hp.
Wounded. You take a wound and fall unconscious. After a turn, you awake with 1 hp. You must retreat from the dungeon as soon as possible. You must rest for a week before adventuring again.
Sacrifice. Your squire/henchman steps in front of the last blow and falls to 0 hp instead of you. They are taken out of the fight, and must recover in a stronghold before adventuring again.
If no squire/henchman, you will die in 1 turn if you receive no healing.
Weak. You are conscious, but weak. You can crawl and whisper, but not perform any tasks that require a roll. At the end of the combat, you regain 1 hp.
Last chance. You are conscious, but weak. If you lay still, you regain 1 hp at the end of combat. If you take any actions, treat them as having rolled a natural 20, but you will die at the end of the round.

Old-school saving throws are one of those things you always want to name and rename until they're perfect (like attributes). One of their virtues is that they tell you what the game is about (Death! Poison! Petrification!). Trying out this set for a little project. They map to the OSE ones.
An OSE style class table for the Knight with the saving throws: Faith, Wit, Grit, Quick, and Counter-spell.

As part of this, I'm adapting some class features into hypo-diegetic rules. For example, knights can wear heavy armor and wield martial weapons because they have the legal right to do so - commoners cannot.

I am once again thinking about this matrix that I made I think formalizing the implied flow of a character and what it means for the setting of your D&D (generic) world is a space worth exploring
A matrix showing the core D&D classes and their implied place in the world before adventuring, during adventuring, and after death or retirement

Factions are such an important part of OSR play because the OSR is full of factions that all hate each other, so we really can really imagine what it's like.

I am once again pleading for any RPG creators who make PDFs to put *something* on them to let me know what game this is for. When I am trying to sort through hundreds (or thousands) of files and I come across handouts.pdf or charsheet.pdf, I'd really like to put them with their parent game.

Reposting for the Friday crowd. A megadungeon is a series of minidungeons. This is one such minidungeon. It's a cool one, I think. Connect it to your dungeons to make them bigger. Plus, you get to see how shitty my handdrawn maps are! (How simple they need to be for your players, I mean.)

a stone staircase spirals up towards us: at the bottom, a figure in a tunic and phrygian cap looks about as he approaches the first steps; a short way up, the figure is sprawled on the steps, his head cleanly sliced off; further up, his hand is vanishing into green sludge which fills a pit that has opened in the steps; a few steps higher, he is pinned in place by a vertical row of spears jutting from holes in the wall; up a few more steps, he has been crushed by a block of stone cut to fill the width of the stairs; almost at the top, only a pair of smoking shoes remain by the metal grille in the floor through which the glow of fire can be seen; at the top, having reached the landing, the figure looks down at his foot as the floor tile he's stepped on has just depressed with a 'click!'
