This trial is a beautiful illustration of the folly of use of NACE in non-inferiority trials of antithrombotic therapy. Paradoxical claim of non-inferiority despite increase in death with clopidogrel monotherapy. This should lead to a moratorium on NACE for NI trials (but sadly won’t)

Great that folks are studying this question, but this non-inferiority trial is non-informative by design. This may be the worst, most irrational NI margin (5% absolute for 3y death) I’ve seen used in a cardiology-adjacent trial so far. A claim of non-inferiority based on this says nothing

“We are heartbroken but also very angry.

Alex was a kindhearted soul who cared deeply for his family and friends and also the American veterans whom he cared for as an ICU nurse at the Minneapolis VA hospital. Alex wanted to make a difference in this world. Unfortunately, he will not be with us to see his impact.

I do not throw around the ‘hero’ term lightly. However, his last thought and act was to protect a woman. The sickening lies told about our son by the administration are reprehensible and disgusting. Alex is clearly not holding a gun when attacked by Trump’s murdering and cowardly ICE thugs. He had his phone in his right hand and his empty left hand is raised above his head while trying to protect the woman ICE just pushed down, all while being pepper sprayed. 

Please get the truth out about our son. He was a good man. Thank you.

I hereby submit my S 2026 syllabus for PHIL 111, Contemporary Moral Issues, for mandatory censorship review.
The syllabus has not changed much since I last taught the course. I have made some minor adjustments to the module on Race and Gender Ideology and to the lecture on Sexual Morality. These topics are commonly covered in this type of course nationwide, and the material is discussed in depth in the assigned textbook (Fiala and MacKinnon, 10th edition). I also ask my students to read a few passages from Plato (Aristophanes' myth of the split humans and Diotima's Ladder of Love).
Please note that my course does not "advocate" any ideology; I teach students how to structure and evaluate arguments commonly raised in discussions of contemporary moral issues.
If you interpret System Rule 08.01 §2.1(b) as prohibiting these topics, I would like to remind you that the U.S. Constitution protects my course content. Texas A&M is a public institution bound by the First Amendment. The Supreme Court has noted that academic freedom is "a special concern of the First Amendment, which does not tolerate laws that cast a pall of orthodoxy over the classroom" (Keyishian v. Board of Regents, 385 U.S. 589, 603 [1967]).
Another relevant precedent is Sweezy v. New Hampshire. Here is Chief Justice Earl Warren for the majority in 1957:
"The essentiality of freedom in the community of American universities is almost self-evident. No one should underestimate the vital role in a democracy that is played by those who guide and train our youth. To impose any straitjacket upon the intellectual leaders in our colleges and universities would imperil the future of our Nation. No field of education is so thoroughly comprehended by man that new discoveries cannot yet be made. We sent in our letter and I know Min. Joly received it. This was not part of the ask. Investing directly in tri-council is a better use of these funds. We already have a recruitment advantage over the US now. We need to value what we already and invest in Canadian research sovereignty

Canadian scientist here. I don't want this. I want you to fund our excellent early career scientists who do great work in Canada. Fund them properly and that will help 1000x more than recruiting handfuls of cherry picked bigwigs from Harvard, Stanford and MIT who will leave after a few years.

FRESH-UP is my favorite trial of 2025 so far (despite some superiority design limitations). My takeaway: In patients with chronic HF, liberalizing fluid intake vs restricting <1.5L doesn't worsen HF symptoms or require higher diuretic doses, among euvolemic patients on good GDMT 1/ rdcu.be/efInN

What a journey this paper took. Hydroxychloroquine was the first (false) hope of the pandemic. Trump touted it and took it himself. Even my hospital at the time, Yale, jumped on the bandwagon, briefly using it. But researchers and patients generated RCT data showing it doesn’t work for COVID.

[citation needed] Nearly every article on MRAs in HF (+++ in the last few days) make this statement without any supporting evidence, which hurts their credibility. What am I missing? Even though the observed rates were lower than assumed (22%), had NI margin been fixed as risk ratio of 1.18 ([22+4]/22), the overall results would still have met NI as RR was 0.90, 0.77-1.05. An excellent example of how a NI trial should be designed and executed. Congrats to trial investigators.

I've previously been critical of non-inferiority (NI) trials in ID, usually because of bus-sized NI margins allowing almost any result to be able to claim NI, but this is a great one. Certainly better than recent examples of NI trials in cardiology, which have become egregiously biased to show NI
