Societies infrastructure to create knowledge is mainly built on the mud, that comes from the trust into organisations with inherent conflict of interests and the means to fabricate any type of raw data. I have ideas but not the funding nor time to get it.

No matter what they decide about clinical significance, not correcting it is definitely not the way to go.

29/ This shows that even a small zombie trial can severely “infect” the overall results of a NMA. Furthermore, this shows that unreliable/zombie trials can be undetected with classic risk of bias analysis.

27/ Nonetheless, we were able to closely reproduce the findings of the three NMAs. !!! Main finding: excluding the zombie trial resolved inconsistencies between the Lancet and Cochrane NMAs and also inconsistencies between direct and indirect evidence in the Lancet NMAs

OK now that I read the excerpt I can actually understand why he talks like this. It's his typical schtick: short answer, mention and diffuse many other attacks on him. Trying to point out great things while critisizing the press. Where is the wow I expected after reading those 4 comments?

If a machine is deterministic& its insides &operations transparent, then I trust its outputs more than any humans. It's the nondeterministic execution of LLM's that stops us. The solution though is right at hand: deterministic validation for nondeterministic generation. Exactly what we do for humans

Amazing story. I thought immediately of a stray H atom, the moment the mass difference was revealed. But not of isotopes. Although it makes absolute sense in retrospect. Using deuterium to change your molecules properties. Ultra advanced pharmacokinetics.

I’ve been told for years that it’s legally and ethically impossible to share medical RCT data. In 2024, the BMJ made sharing anonymised data mandatory, and now lots* of folk do it. Interesting how the barriers evaporated like that, isn’t it? *not everyone, compliance problems exist.

And does it shift the Systematic reviews outcomes? This is exactly the kind of stuff expected to occur to either unintentionally shift results (studies hiding behind assumed truths) or sway them on purpose (conflict of interests). Its even in cardio. That's the stuff James Heathers should work at.

Isn't the idea of a preregistration to not change the goalposts anymore after shooting? I mean sure in such a case you can do it all beforehand and make it seem like you did it afterwards to completely circumvent the purpose of prereg. But the naive mind might catch some real good will with this.

Peter Doshi did sponsor individual patient level data reviews with RIAT to find deeper hidden problems in clinical trials, below the optimistic summary. FDA & EMA rarely do it. It takes a lot of time/ about a year to do it for 1 person. But problems can be found that change results.
000
Josef M Klein @resurgequality.bsky.social · 30/07/2026
Probly needs to be rerun 4-5 times & the positives need to be reviewed & the false negative rate tested with existing IPLD-reviews bc of the high error rate in retrieval based tasks. But when one is there & knows how offen it has to be rerun 2 get low error we can automate this rare necessity.

Josef M Klein @resurgequality.bsky.social · 30/07/2026
Well take Claude code assistant or open claw with some LLM you want, get the Individualität Patient level data of a trial and review it for consistency (like done by RIAT etc) to find deeper problems in clinical triala, not via Hand but automatically.

People should really be careful with those kind or jokes. They basically deny that any safety testing is needed before any harm is known. Rather point to data that shows safety than to deny the need for safety. Or go buy your muuuch cheaper Chinese car that had 0 safety testing.
