The Effectiveness of Nirsevimab on Antibiotic Use in Children Using Target Trial Emulation
Nirsevimab is a monoclonal antibody with demonstrated efficacy in preventing respiratory syncytial virus (RSV) infections in infants. Because acute respiratory tract infections (ARTIs), including viral infections, often lead to antibiotic prescribing, we aimed to determine the effectiveness of nirsevimab on antibiotic prescribing among infants with medically attended ARTIs.MethodsA primary care attendee cohort was established using electronic health record data from 32 diverse practices across a healthcare network to emulate a target trial comparing nirsevimab to no treatment among infants <8 months. Eligible infants were seen at a primary care practice within 14 days of life. Primary outcomes included time to first antibiotic prescription for (1) outpatient ARTIs, (2) outpatient encounters for bronchiolitis, and (3) RSV-related hospitalizations. Nirsevimab effectiveness was calculated as 1 minus the risk ratio for each primary outcome.ResultsAmong 15 341 patients who met inclusion criteria, 7413 received nirsevimab. Compared with no treatment, nirsevimab administration led to a 14.4% (95% CI: 7.8%, 20.6%) reduction in antibiotic prescribing for outpatient ARTIs, a 40.3% (95% CI: 25.1%, 52.5%) reduction in antibiotic prescribing for outpatient bronchiolitis, and a 69.4% (95% CI: 4.8%, 90.1%) reduction in antibiotic prescribing for RSV-related hospitalizations.ConclusionsNirsevimab administration in primary care reduced antibiotic use for ARTIs in both ambulatory and inpatient settings.

In 2024, I wrote a story for Texas Monthly about Michelle Haas, whose complaints resulted in the removal of books about slavery from historic Texas slave plantations. Haas sued us, alleging defamation, and was represented by the preferred lawyer of billionaire J.P. Bryan. Today, we won on appeal.

Jay Bhattacharya is a health economist who dislikes virology because he thinks it’s reckless & dangerous to study pathogens or develop vaccines. I’m a virologist who dislikes this take on health economics because I think it’s reckless & dangerous to take $8.2 trillion in life value from Americans.
Azouley et al3 estimate that a 33% reduction in NIH funding would be associated with a 15.3% reduction in patents associated with new drugs. We assume, like them, that this will lead to 15.3% fewer new therapies.3 Although we do not quantify this here, cuts already made and proposed future staffing cuts at the US Food and Drug Administration may further slow the pace of drug approvals.

New therapies lengthen lives.4,5 Buxbaum et al6 estimate that new medical therapies generated 48% of the 3.3-year increase in life expectancy in the US between 1990 and 2015. If biomedical research remains as important in the future as in the past, a 15.3% reduction in new therapies would lead to a reduction in life expectancy of 0.24 years per person over the next 25 years. In a population of more than 340 million, this reflects 82 million fewer years of life. Economists use various methods to estimate the value of life and typically find that years of life are valued at roughly between $100 000 and $200 000 per year.6 Even using the lower value in calculations, the lost health from the NIH cuts translates into more than $8.2 trillion ($100 000 × 82 million years).

These losses would be weighed against the gains that emerge from reduced public spending. Over 25 years, the proposed annual savings of $20 billion amounts to $500 billion in budgetary reductions. This pales in comparison to the $8.2 trillion
