Outcomes continue to improve for myeloma since I started back in 1999, nice to see the improvement in context with other cancers. The top cancer success stories of the past 50 years, by the numbers www.nature.com/articles/d41...

Outcomes have improved drastically in the United States for some forms of the disease, but those for others continue to lag.

Congratulations to Dr. Nizar Bahlis (U of Calgary) who is the 2026 recipient of the International Myeloma Society Anderson Award. A well deserved honour that reflects his impact on Myeloma care. #IMS26

Jan1, July 4, and December 25 outliers must be a reporting variation or is the induction rate higher the day before to get people out of the hospital or do holidays delay the onset of labor? All the same didn’t realize there was such a variation over the year

Thinking about how plants and animals, who independently evolved multicellularity, both landed on the spatial expression of transcription factors as the way to figure out where all the parts go.
A diagram showing the MADS domains patterning a flower. Gene expression goes from the outside in with A,B,C,D, and E domains. E is expressed in all cells. In some cells, there is A and E expression, those cells develop into sepals. Cells with A, B, and E expression become petals. Those with B, C, and E become stamens. Those with C and E become carpals, and C, D, and E cells become ovules.Hox gene expression in the fly. The genes are shown in order on the chromosome and appear in the same order in the fly. The first gene patterns the mouth, the second the forehead, the third, the back of the head, etc.

For some reason Illumina has been targeted in the tariff responses from China. Not sure why. Suspect due to highly competitive instruments from Chinese companies are largely blocked from sales in USA or they know it’s a connection to the administration that can be leveraged🤷‍♂️

Without your commitment and vision that precision medicine needs to be deployed to each and every myeloma patient I doubt we would be here today. On behalf of the patients that have already benefited and will benefit in the future, thank you!

2) Our myeloma team at City of Hope, lead by the amazing Amrita Krishnan. I’m so proud to be part of this amazing team. 3) Our team in the TGen clinical lab that build onto our amazing ALTseq technology that was already providing rapid WGS for AML to make this dream a reality

Last there are many people who I need to thank 1) The Hollingsworth and Nason families, it was through your confidence in us with the management of Jay’s case that our team learned the true power of this approach and none of this would have been possible without your generous support Susie

5) Multiple patients with loss of BCL2L11/BIM and KDM6A/UTX, two of the things I never want to see in a myeloma tumor
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
4) Patients with bi-allelic inactivation of CDKN2C due to mutations on the second allele. The IMS/IMWG CGS only considers bi-allelic deletions as high-risk officially. We really need to amend that at a minimum.
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
In the future I hope we will talk about target gene dysregulation not the specific event that caused it as that confounds our understanding today. 3) More patients than I can count that have complete loss of RB1, the leading marker for the high-risk PR population

Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
2) Patient with MAF overexpession due to a t(1;16) bringing the TENT5C enhancer into proximity with MAF. What is the high risk marker t(14;16) or overexpression of MAF? The number of MAF family members and NSD2 overexpression cases by non-IgH enhancers will surprise many in the myeloma community.
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
Not considered high risk because no FISH papers tested for it while we showed in CoMMpass its just as common as the t(14;20)/IGH::MAFB event that is considered high risk and all three MAF family member translocation cluster together by RNA clustering
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
Having access to these kinds of results also opens up your eyes to features we didn't add to the new IMS-IMWG high risk definition 1) Patient with a t(8;14)/IGH::MAF translocation with 1q21 gain.
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
5) Patient with complete loss of TP53 due to a deletion and inversion when FISH said it was normal due to the split of the remaining TP53 allele by the inversion and the FISH probe partially sitting on both sides 6) Multiple CDKN2C bi-allelic deletions that were missed by FISH
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
4) CD38 complete loss due to a deletion and a translocation that broke the remaining copy in half. This is something WGS can do and most panels will completely miss
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
This is true personalization of therapy when the tumor board discussion concludes that even though the patient has a mutation on the remaining copy it would only block ide-cel function and all other BCMA therapies are still an option for this patient
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
3) Identification of a patient with a BCMA deletion and mutation post Ide-cel, where the mutation was predicted to inhibit only Ide-cel in an alanine screen.
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
2) Identification of a t(11;14) positive patient that got access to a BCL2 inhibitor trial, whose t(11;14) was missed because of an external FISH reflex strategy. WGS tests for everything all at once so no reflexing, no decisions on which tests to skip when sample is limiting
100
Jonathan Keats, PhD 🇨🇦 @pedalheadphx.bsky.social · 07/05/2026
I could post for days on the things we have already seen but here are a few highlights 1) Complete loss of BCMA in multiple clones with 11 different deletion events detected

I strongly believe it will lead to more accurate prognostic models, advancement of new therapies (I'm looking at you RAS inhibitors), optimization of immune based therapy with antibodies (naked or ADC), bi-specifics, tri-specfics, and CAR-T.

It also represented a fantastic collaboration between the TGen clinical lab team, City of Hope Pathology team and our clinical teams to complete the validation efforts. The transition to whole genome sequencing from FISH is going to fundamentally alter how we view each myeloma case.

For those who still work with SPSS files, but want to import them into #rstats If you want to retain user defined missing values in your data (-98 = refused), don't forget to add the "user_na = TRUE" argument if you are using haven::read_sav(). Otherwise, user-defined missing values import as NA.
