This is interesting. You get assigned to being born at a random place and time in history, to see what a typical person's life might have been like then. Unfortunately I was born in 1403 in the valley of Mexico and died at age 4 from dysentery.

Stroke offers a unique window into how the brain constructs bodily awareness — and how its disruption shapes recovery in ways we still poorly understand.

Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
Priorities for future work: • mechanistic studies of interoception–allostasis • standardised interoceptive assessments • integration with rehabilitation science • targeted interventions for physiological dysregulation

Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
We argue that stroke research needs to move beyond isolated symptoms and adopt an allostatic–interoceptive framework that links bodily sensing, regulation, and recovery trajectories.
100
Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
This is a problem because recovery is not just motor relearning or compensation — it is physiological regulation. If the brain cannot accurately sense the body, prediction and regulation break down.
100
Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
The biggest gap: allostatic load. Despite strong links to cognition, recovery, and health disparities, it has been almost entirely neglected in stroke research.
110
Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
Neuroanatomy plays a role — but the evidence is uneven. Deficits are most often linked to insula and somatosensory lesions, yet lesion mapping is inconsistently reported and rarely theory-driven.

Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
These are not peripheral symptoms. Interoceptive impairments are often associated with: • poorer functional recovery • cognitive dysfunction • altered body ownership/awareness • reduced independence and participation
110
Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
Key finding: interoception is frequently impaired after stroke, across multiple domains, including: • cardiac & respiratory awareness • temperature & affective touch • fatigue & energy regulation • hunger & bladder awareness

Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
We conducted a scoping review of the literature on interoception and allostatic load after stroke. 👉 39 studies met inclusion criteria 👉 Only one directly assessed allostatic load That imbalance matters.

Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
Interoception = sensing the internal state of the body (heartbeat, breathing, temperature, hunger, fatigue). Allostasis = how the brain predictively regulates these states to maintain stability. Both are central to recovery — yet rarely studied together in stroke.

Paul Jenkinson @paulmjenkinson.bsky.social · 20/01/2026
Many post-stroke symptoms don’t fit neatly into “motor” or “cognitive” boxes — fatigue, altered body awareness, disrupted breathing, blunted hunger, feeling disconnected from the body. This paper explores what happens to interoception and physiological regulation after stroke 👇

My reviewing style has changed over time. Rather than litigate every little thing, and pushing my own ideas, I focus only on 2 things: (1) Are the claims interesting/important? (2) Does the evidence support the claims? Most of my reviews these days are short and focused.

7/ 🧠 Our conclusion: Stroke disrupts internal bodily regulation in ways that matter—but the field is just scratching the surface. We call for: ▪️ Standardised interoceptive measures ▪️ Longitudinal studies ▪️ Mechanistic insights ▪️ Rehab strategies targeting interoception
100
Paul Jenkinson @paulmjenkinson.bsky.social · 28/07/2025
6/ 📉 Allostatic load—essentially the long-term physiological cost of stress—has not specifically been characterised and studied post-stroke. The one study that did measure it found that higher load predicted worse cognitive outcomes, especially in marginalised populations.
100
Paul Jenkinson @paulmjenkinson.bsky.social · 28/07/2025
5/ 🧠 Interoceptive changes were often linked to poorer outcomes: ▪️ Lower functional recovery ▪️ Cognitive and emotional challenges ▪️ Disturbed body ownership Insular and somatosensory lesions were most commonly implicated.
100
Paul Jenkinson @paulmjenkinson.bsky.social · 28/07/2025
4/ What did we find? Interoceptive deficits post-stroke are common. Disrupted domains include: ✔️ Heartbeat detection ✔️ Breathing awareness ✔️ Temperature and touch perception ✔️ Fatigue and hunger ✔️ Bladder and muscle sensation
100
Paul Jenkinson @paulmjenkinson.bsky.social · 28/07/2025
3/ 🔍 We reviewed 39 studies: ▪️ 38 focused on interoception after stroke ▪️ Only 1 (!) specifically characterised and examined allostatic load. That gap alone speaks volumes. We need to better understand the body’s complex internal regulation systems in stroke recovery research.
110
Paul Jenkinson @paulmjenkinson.bsky.social · 28/07/2025
2/ Why this matters: Interoception plays a key role in regulating internal bodily states. When the brain is damaged by stroke, this internal sense can be thrown off balance—impacting recovery, cognition, emotion, and quality of life. But how exactly? No one had mapped this—until now.

Paul Jenkinson @paulmjenkinson.bsky.social · 28/07/2025
🧵1/ New preprint alert! We just pre-printed a scoping review exploring how stroke disrupts interoception—our sense of the internal body—and its possible effects on allostatic load (the physiological toll of chronic stress). 🧠🫁🫀🧵 Read it here 👉 osf.io/preprints/ps...

7/ Implications? 🛠️ Interoceptive-based therapies should focus on regulation, self-trust, and metacognition—not just improving accuracy. 🧪 And future research should carefully choose tools that reflect adaptive vs. maladaptive interoceptive processes.
110
Paul Jenkinson @paulmjenkinson.bsky.social · 25/07/2025
6/ We interpret this in line with Allostatic Self-Efficacy theory—depression may emerge when people lose confidence in their ability to regulate internal states. 🧩 These findings support a multidimensional model of interoception, where not all interoceptive tools are created equal.
100
Paul Jenkinson @paulmjenkinson.bsky.social · 25/07/2025
5/ Between-group studies (depressed vs. controls) showed stronger effects than symptom-based correlations in healthy samples. And interestingly, not all self-report tools agreed—MAIA showed strong links; others (like BPQ, BRQ) showed positive or mixed associations.
100
Paul Jenkinson @paulmjenkinson.bsky.social · 25/07/2025
4/ This suggests that how people interpret and regulate their internal states—not just how accurately they detect them—is crucial in depression. Regulatory & metacognitive interoceptive processes may be promising targets for intervention.
100
Paul Jenkinson @paulmjenkinson.bsky.social · 25/07/2025
3/ We found a significant negative association between self-reported interoception and depression (r = -0.17). That is, poorer interoception = higher depression. The effect was strongest for: 🧘‍♂️ Self-regulation 🎯 Attention regulation 🙏 Trusting bodily signals
