Glad to see the ID community coming together and issuing a call to action/recognizing our duty to address misinformation and attacks on public health!

This is the legacy of MAHA right now but the scars left on public health are deep; also hearing the stories and living them in the measles experiences right now…

I thought the same—I initially thought it was an advertisement (I flipped over from reading the news re: measles controversies and am so used to all the ads!)

Cefoxitin is a second gen and I’m not just talking about c diff, but nausea and other ADE. Also nursing time and plastic waste for all those infusions.

Also all the toxicities associated with gent and levels/monitoring. There are also dosing guidelines variations for OB that vary by practice site/cultural prescribing practices—>25 different regimens used for same indication in one survey. Spectrum is also key, considering lack of micro in IAI.

easy to stick the 2g cefoxitin in the Pyxis, but the gent has to be ordered, mixed, sent to the floor…inefficient! Honestly I think that’s the key to all the work we’ve done around cefoxitin—getting effective drug in patient is obviously the most important thing (extrapolate from sepsis!)

We also had the hard stop for sxs (and side note, took out “voided” as specimen!) but that last 24h before ER panicked. But we tried. I’m about to put this all into a paper. Tons of education w/ all of this. Trying something similar at my new institution. Happy to send better screenshots via email!

Ok so I don’t have a screenshot on my phone, but we added verbiage to the order panel (forced all UA ordering through this panel—UA, UA with reflex, then ASB peeps got UA+cx and must be paired bc they can contaminate specimens too)

Can AMS survive war? In a Ukrainian maternity hospital, low-cost stop-orders shifted SSI ppx from ceftriaxone (72%→1%) to cefazolin (0%→44%). Result: 0% unjustified post-op dosing, 50% lower LOS & savings of $1,040/case. A proven model for institutional resilience in crisis.

☀️ Morning, friends Inspiration from one of our greats—Dr. Bill Foege, whose leadership helped eradicate smallpox. Near the end of his life, he reminded us: public health can be great again. And he believed in all of us to carry it forward.
Quote from Dr. Bill Foege: "We will live through this drought of values, principles, and facts and again apply our talents to improving global health and happiness. Do not back down."
